Summary:
Actuality. According to the medical and social significance, cardiovascular pathology is in the first place among non-communicable diseases. Despite the fact that more than 40 years have passed since the first percutaneous coronary intervention (PCI), the optimal regimen of anticoagulant therapy is a serious problem for clinicians Purpose: to analyze data on bleeding in elderly patients with acute coronary syndrome who have undergone percutaneous coronary intervention, receiving anticoagulant therapy Materials and methods: analysis of literature data on search words - aging, hormones, endocrine system, old age, senile age, adrenal glands, hypothalamus, pituitary gland glucocorticoids, dehydroepiandrosterone for 2005 - 2020 in computer databases: PubMed, Scopus, Medical-Science, Elibrary, Web of Science, Ceeol. Results. Anticoagulant therapy is a standard of care for patients with coronary heart disease with percutaneous coronary intervention. Significant uncertainty remains regarding this therapy in patients with a high risk of bleeding, especially in the elderly. As a rule, most of the studies did not assess the risk of bleeding, but focused on ischemic risk. However, in recent years, bleeding has been recognized as a significant factor in morbidity and mortality in patients undergoing percutaneous coronary intervention. At the same time, at the moment there is no single classification of bleeding, and the presence of many of its options makes it difficult to interpret clinical data and the choice of patient management tactics. Conclusions. Вleeding in elderly patients undergoing percutaneous coronary intervention is a serious iatrogenic problem. The unified classification of bleeding remains unresolved. Insufficient studies in geriatric patients.
