GERONTOLOGICAL CONSULTATION ONLINE IN THE CONTEXT OF THE COVID-19 PANDEMIC

Summary:
Actuality: The era of the COVID-19 virus pandemic leads to an exacerbation of many chronic diseases in people of different ages, as well as an increase in the prevalence of anxiety, depression, panic, loneliness, and obsession. In this regard, we have developed a project called gerontological consultation online. Gerontological consultation is a unique Russian project designed to help people of different ages find the right behavioral program to achieve healthy longevity. It is also aimed at solving people's health problems during the period of convalescence after suffering COVID-19. Gerontological consultation provides an opportunity, based on an individual interview, to develop programs for active longevity in the context of a pandemic that is suitable for this particular person, including advice on providing social assistance in a difficult life situation, and advice to relatives who help a person recover from a disease. Aim: To study the effectiveness of online gerontological consultation in the context of the COVID-19 pandemic. Material and methods: The research was based on the therapeutic polyclinic Department at the Clinical hospital No. 85 in Moscow. In the first part of the study, we identified the most common health problems in patients of different ages during the period of convalescence after COVID-19. The study sample consisted of 79 middle-aged people (51.8+1.2 years) and 72 elderly people (68.1+1.3 years). To determine health problems, we used scales used in gerontological practice. After analyzing the data obtained, we applied online gerontological consultation for patients who had undergone COVID-19 and were in the convalescence phase. The control group, whose patients did not have a gerontological consultation, consisted of 57 middle-aged people (52.3+1.2 years) and 61 elderly people (68.3+1.3 years). The main group of patients who received gerontological consultation consisted of 58 middle-aged people (51.2+1.2 years) and 59 elderly people (67.8+1.3 years). Gerontological consultation was conducted once a week for 3 months. Results: The use of gerontological consultation in patients of different ages in the convalescence phase after COVID-19 leads to a significant decrease in the prevalence of anxiety-depressive syndrome, obsessive-compulsive syndrome and an increase the resilience in middle-aged patients, a significant decrease in the prevalence of anxiety-depressive syndrome, loneliness syndrome, and a less significant, but statistically significant increase the resilience in elderly patients. This distribution of solutions to health problems and improvement of bio-psycho-social status between middle-aged and elderly patients is probably associated with a higher adaptive capacity of the body of middle-aged patients compared to elderly patients. After applying online geriatric consultation in patients of middle age in the phase of convalescence after suffering COVID-19 showed a significant significant increase is physical activity, commitment to a balanced diet, application of herbal medicine, the elderly patients showed a significant a significant increase in additional use of nutrients, trace elements, vitamin, commitment to conservative therapy, to compensate for somatic diseases, use of herbal medicine, this led to a decrease in ambulance departures and normalization of objective and laboratory indicators of somatic status (achievement of target blood pressure figures, normalization of total cholesterol and glycosylated hemoglobin). Conclusions: In the course of the study, we identified the main directions of gerontological consultation, which included educating patients and their environment to a healthy lifestyle, proper adequate level of physical activity, postulates of rational nutrition, getting rid of obsessive thoughts, teaching the patient's relatives to help get rid of odinovestvo syndrome, the reasons for using medications necessary for correcting somatic status problems, explaining the concepts of "target blood pressure numbers", glycosylated hemoglobin, lipid profile indicators, explanation and recommendations on the need for additional nutrients, trace elements, vitamin therapy, herbal medicine.
