ROLE OF CENTRAL REGIONAL HOSPITALS IN ORGANIZATION OF PRIMARY MEDICAL - SANITARY CARE TO THE POPULATION

Relevance. Changes in the socio-economic situation in the country affected the development trends of central regional hospitals, which led to significant changes in the network of medical organizations, the role of central regional hospitals (hereinafter - CRH) and regional hospitals (hereinafter - RH) in the process of organizing medical care Purpose. Assessment of the dynamics of indicators characterizing the work of outpatient departments of the CRH and RH: the number of visits, attendance rate, personnel and material and technical support of these departments. Material and methods. Data from Rosstat, Ministry of Health of Russia, FSBI "TsNIIOIZ" of the Ministry of Health of Russia. Used analytical, statistical methods and comparative analysis. Results. Over the period from 2014 to 2019 in the Russian Federation against the background of a decrease in the number of CRHs and RBs by 7.6% with an increase in their planned capacity by 0.5%, the total number of visits to them decreased by 10.3% as a whole, and per one resident - by 10.7%. In general, all visits to the Central Regional Hospital (to the polyclinic and at home) across the Russian Federation decreased by 15.8%, and per capita the number of visits decreased from 2.1 to 1.7 (by 16.2%). A decrease was recorded both in highly urbanized regions of the Russian Federation, and in regions with a large share of the rural population. On the contrary, the number of visits to RHs throughout the Russian Federation as a whole in the same period increased by almost 10 million, or by 14.4%. The number of home visits in CRHs and RHs decreased. The rate of visits per capita to district hospitals increased from 0.47 in 2014 to 0.54 in 2019. The number of full-time medical positions in polyclinic units in total in the CRHs and RHs decreased by 2.3 thousand, the number of employees was reduced by more than 9 thousand. Discussion. A significant decrease in the number of visits and attendance at CRHs and RHs was revealed. The rate of decline in the number of visits to central regional hospitals and district hospitals in some constituent entities, including those with a high proportion of the rural population, far exceeded the rate of decline in visits to these organizations as a whole in the Russian Federation, which indicates that the reforms in these constituent entities were not residents of villages and small towns. Against the background of a decrease in the number of hospitals of this type by 7.9%, possibly due to their merger, the number of full-time medical posts in the CRHs and RHs decreased during the analyzed period by only 1.9%. Findings. Rational placement of the network of medical organizations "first contact", increasing their role in the process of organizing medical care in the territory of municipalities is an essential condition for improving the availability of primary health care.
