PROBLEMS OF ORGANIZING PRIMARY HEALTH CARE IN CONDITIONS OF PERSONNEL IMBALANCE OF DISTRICT PHYSICIANS AND THE WAYS OF THEIR SOLUTION (literature review)

Summary:
Introduction. The effectiveness of the healthcare system, the availability and quality of medical care largely depend on the performance of medical personnel. Therefore, along with the introduction of new medical, digital, information technologies, it is necessary to improve the organizational technologies for providing medical care. This article discusses promising directions for the development of the organization of primary health care for the adult population. The purpose of the study is to analyze the problems of organizing the provision of primary health care to the adult population in the conditions of a personnel imbalance of local general practitioners and to identify promising directions for its development, taking into account Russian and foreign experience. Materials and methods. An analysis was made of 38 sources of literature, as well as materials of scientific conferences published over the period from 2012 to 2022, devoted, among other things, to the problems of staffing and the study of the workload on the medical staff of primary health care, the issues of increasing the role of nurses in the district service , the works devoted to the issues of organizational decisions in Russia and abroad in the provision of primary health care have been studied. Results. Despite the measures taken to provide the industry with qualified personnel, we can talk about the continuing problem of shortage and imbalance of medical personnel in primary health care. Meanwhile, a number of authors note the use of new organizational technologies as a promising and optimal solution to ensure the availability of medical care to the attached population in the current situation. Discussions. Training of qualified medical personnel, building up human resources, developing a network of medical organizations are necessary conditions for providing the population with medical care. However, the introduction of new organizational technologies for the provision of medical care, including those based on the brigade form of labor organization, help to solve the problem of access to medical care in the context of a continuing imbalance in personnel. In general, the problems of primary health care remain unresolved, but even now it is possible to identify promising areas for its improvement. Conclusion. The brigade form of labor organization in the provision of primary health care to the adult population has not yet been reflected in regulatory legal acts. It is necessary to ration the work of a district nurse working as part of a team at a medical site, study the impact of specific conditions for the provision of medical care on the size of the team, and determine the functional responsibilities of its individual members. The prospect of introducing a brigade form of labor organization of medical workers providing primary health care on an outpatient basis lies primarily in the fact that, in the conditions of the existing staff shortage, it is possible to determine the optimal need for primary health care specialists and ensure the availability of primary health care.
