QUALITY ASSESSMENT OF THE SCREENING TEST FOR SUDDEN CARDIAC DEATH PREDICTORS

Introduction. The problem of sudden cardiac death is one of the key problems in the field of railway medicine, since the safety of the life of a large number of people, the safety of infrastructure facilities, transport and material assets depend on the health of workers providing the transportation process. Aim. To assess the quality of a screening test for predictors of sudden cardiac death using statistical quality control of the verified diagnostic test (having binary outcomes). Materials and methods. In 2008-2013 period, a follow-up of 22 items of the natural group of initially healthy 7,959 men (employees of locomotive crews of the Trans-Baikal Railway) aged 18-66 years was conducted. As a result of the study, such predictors of sudden cardiac death as excessive alcohol consumption, reduced glomerular filtration rate, psychosocial stress, III degree obesity, arterial hypertension, and age range of 34-66 years old were established. DiagStat evaluated their predictive power when used in screening tests for the prognosis of the disease under review. Results. Diagnosis of reduced glomerular filtration rate (below 60 ml/min according to MDRD or Cockcroft-Gault formula) predicts the likelihood of 0.5-78.9% of a patient’s sudden cardiac death. Diagnosis of excessive alcohol consumption suggesting systematic daily single alcohol consumption above the WHO safe limit, i.e. more than 2 standard doses of alcohol per day with 1 dose equal to 13.7 g [18 ml of ethanol] in terms of alcoholic beverages predicts a probability of 0.5-17.9% sudden cardiac death in men. In the absence of a psychosocial stress, III degree obesity, arterial hypertension and with the age range excluding those aged 34-66 years, probability of sudden cardiac death is not predicted in 99.6-99.9% of cases. Conclusion. It is necessary to purposefully detect such indicators as reduced glomerular filtration and excessive alcohol consumption in workers of locomotive crews before deciding on their admission to train operation, since the detection of these predictors of sudden cardiac death in this category of workers is not stipulated by the existing regulations. As for the remaining predictors of sudden cardiac death, their targeted search should also be carried out considering the existing techniques and the ease of their detection.
