DETERMINATION OF LIMB STRENGTH IN PREDICTING THE RISK OF THE SYNDROME OF FALLS

Summary:
Introduction. More and more studies are devoted to the study of risk factors for the development of chronic diseases associated with falls in the elderly. Such studies are necessary for the development of a prevention program. Assessment and intervention strategies tend to be multicomponent and difficult due to the complex and multifactorial nature of falls. Purpose of the study. To study the possibility of determining the strength of limbs in predicting the risk of the syndrome of falls. Materials and methods. The study included 172 patients aged 65 to 74 years, who underwent a comprehensive assessment of physical activity according to the following parameters. To assess the parameters of stability and gait, the Tinnetti questionnaire "Assessment of motor activity in the elderly" was used with the selection of patients with normal indicators of motor activity, mild, moderate and significant disorders. The volume of physical activity was determined anamnetically in min/week. The assessment of the functions of the muscles of the upper extremities was determined using carpal dynamometry. The study of the strength of the muscles of the lower extremities was carried out using dynamometer stands. Information about the syndrome of falls was collected according to anamnesis. Results. Elderly patients with vascular diseases of the lower extremities with a comparable clinical picture of diseases and the functional state of the vessels represented a heterogeneous group of patients with a uniform distribution according to the degree of motor activity disorders: 26.2% of them had normal motor activity indicators, 24.4% - mild motor activity disorders (34-38 points), 27.3% - moderate disorders and 22.1% - significant violations of motor activity. Motor activity disorders were formed both due to gait disorders and stability disorders. The revealed violations of motor activity naturally led to a decrease in the volume of physical activity in patients. A decrease in the volume of physical activity led to a deterioration in the indicators of muscle strength of the lower and upper extremities. Moderate and significant motor activity disorders in elderly patients with vascular diseases of the lower extremities were associated with the syndrome of falls. Conclusion. The syndrome of falls in elderly people with vascular diseases of the lower extremities had a direct relationship with a decrease in limb strength, which makes it possible to use the definition of limb strength as a criterion for predicting the risk of the syndrome of falls.
