RISK-ORIENTED APPROACH TO ASSESSING THE ORGANIZATION OF MEDICAL CARE IN THE PROFILE OF TRAUMATOLOGY AND ORTHOPEDICS AT THE LEVEL OF THE SUBJECT OF THE RUSSIAN FEDERATION

Summary:
Introduction. In modern conditions, risk management is one of the effective methods of managing the health care system. At the same time, management decisions require reliable medical and statistical information, the main source of which is the form of federal statistical observation. However, they contain a limited amount of information, which reduces the possibility of identifying organizational risks. The goal is to assess the possibility of using a risk-oriented approach when analyzing the organization of medical care in the profile of "traumatology and orthopedics" at the level of a subject of the Russian Federation. Materials and methods. The study applied analytical, statistical and sociological methods, determined the criteria for inclusion in the study of medical organizations, typed the population into groups by the nature of resettlement, and conducted an organizational experiment. Results. Depending on the nature of resettlement and the conditions for providing medical care in the Stavropol Territory, 4 groups of the population were allocated. It was established that the IV group of the population has the least provision with specialized beds. To identify organizational risks, the "Passport of the Trauma and Orthopedic Service of a Medical Organization" was developed, which includes an expanded amount of medical and statistical information. Discussion. During the study, the principle of voluntary reporting was implemented. Of the 35 medical organizations, only 25 provided completed "Passports." An independent analysis of the information obtained found that in 2019, 19.7% of patients hospitalized in trauma beds correspond to the profile of medical care "neurosurgery." Three medical organizations (population group IV) that do not have full-time positions of an orthopedic traumatologist treated 547 patients with musculoskeletal pathology. Conclusions. The use of a risk-oriented approach with division of the population into groups by the nature of resettlement in the analysis of indicators of medical care provides a comprehensive assessment of the organization of medical care in the profile of "traumatology and orthopedics," confirm the effectiveness of the use of voluntary reporting for independent analysis and subsequent management decisions on the management of the health care system at the level of a constituent entity of the Russian Federation.
