ANALYSIS OF THE DYNAMICS OF STAFFING OF MEDICAL UNIVERSITY CLINICS

Summary:
Introduction. Providing medical organizations with qualified personnel is currently an urgent problem and task of the modern healthcare system. It is no coincidence that this direction, along with others, is included in the federal national project "Healthcare" being implemented in the country. Healthcare organizers face an important task — the development of a set of measures to reduce the shortage of medical personnel, increase the availability and quality of medical care. Special attention is required to maintain the high personnel potential of medical university clinics, since this indicator directly affects not only the effectiveness of medical care, but also the conduct of research and teaching work. Purpose: to analyze the dynamics of staffing of medical university clinics in modern conditions. Materials and methods. The clinics of the medical university – the federal state budgetary educational institution of Higher education (FSBEI HE) of the Samara State Medical University (SamSMU) (hereinafter referred to as the Clinics of SamSMU) were selected as the research base. Clinics are the largest division of SamSMU, representing a medical, scientific and educational complex for the provision of mainly specialized medical care in inpatient and outpatient settings in a wide range of specialties. The analysis of the staffing of the Clinics of SamSMU was carried out on the basis of information in the reporting forms of federal statistical observation No. 30 "Information about a medical and preventive institution" and No. 17 "Information about medical personnel", followed by the calculation of the corresponding indicators. The observation period was eight years, from 2015 to 2022. Statistical and analytical research methods were used. Results and discussion. The number of all full-time positions in Clinics for the period from 2015 to 2022 remains stable, while the number of all occupied positions increased by 17.3%. The structure of full-time and occupied positions is changing dynamically due to an increase in staff units with higher education and a decrease in staff with secondary specialized education. Staffing of full-time positions of doctors, nursing staff, junior medical personnel is below optimal values. There is an increase in the staffing indicators of Clinics in 2020 during the operation of the covid hospital. Since 2021, the trend towards a decrease in human resources indicators has returned, which is more pronounced among middle and junior medical personnel. The ratio between the number of doctors and nursing staff remains very low, which tends to further decrease (1:1.6 in 2015 and 1:1.1 in 2022. Less than 40% of doctors and less than 36% of people with secondary medical education have a qualification category. Conclusion. In the clinics of the medical university, there is a problem with staffing, especially specialists with secondary education. Attention should be paid to timely training of personnel with the involvement of young specialists and mentoring. The growth in the number of doctors, secondary and junior medical personnel in 2020 confirmed the effectiveness of financial incentives to work. One of the solutions to this problem may be the additional involvement of graduates of higher and secondary educational institutions in the target set. It is possible to increase the motivation of employees for certification by determining additional incentive coefficients for the presence of a qualification category with a high assessment of the quality of work of medical workers, taking into account their participation in clinical trials and the availability of scientific degrees and titles.
