INFORMED VOLUNTARY CONSENT - IMPLEMENTATION OF PATIENT-ORIENTED TECHNOLOGIES IN MEDICAL ORGANIZATIONS

Summary:
Introduction. With the development of medical science and the advent of new technologies for diagnosis and treatment, the need has increased to more clearly inform patients before visiting a doctor or upcoming diagnostic and therapeutic procedures about the required amount of examination, the possible risks of using certain methods of treatment and diagnosis, and their health consequences. The purpose of the study is to study the attitude of various groups of patients to the use of IVC in medical practice, taking into account the factors influencing the formation of responsibility for the use of this document in the work of patients and medical workers. Materials and methods. The study was conducted on the basis of the Moscow State University named after M.V. Lomonosov, city polyclinic and perinatal center. In the work, one of the methods of social survey was used - questioning. For the convenience of respondents, the questionnaire was offered in two forms - in a Google form and on paper. For an objective assessment of the process of familiarizing patients with the IVC, a researcher's checklist was developed, which made it possible to clarify the conditions and time for familiarizing patients with the document, establish personal contact and answer respondents' questions, receive suggestions from them on creating conditions in the MO to improve communications between medical workers and patients. Results and discussion. The amount of information contained in the IVC was considered sufficient by 71.4% of the respondents, 28.6% noted the need to increase the amount of information on ongoing medical interventions, especially, possible complications and their prevention. The analysis of the questionnaires showed that patients of polyclinics got acquainted with IVC: in the registry - 36.1%; in the waiting room - 26.5%; in the doctor's office - 37.4%. Therefore, 64% of respondents noted the need for the presence of a medical worker and the availability of comfortable conditions when getting acquainted with the IVC, where you can concentrate and carefully read the proposed text. 71.7% of patients noted that they prefer to ask questions to the attending physician and sign the IVC only after receiving answers to all questions. 79.3% of patients noted the high level of communication skills of the personnel of medical organizations and were satisfied with communication with employees. 54.4% of respondents formed a trusting relationship with medical workers after applying for medical care, which implies repeated visits and observation in these medical organizations. The analysis of the investigator's questionnaire confirmed that the majority of patients (71.4%) carefully read the informed voluntary consent and spend three to five minutes on it. At the same time, 24.3% of respondents got acquainted with the IVC for less than two minutes, practically without reading it, 4.3% - got acquainted with the document for more than 5 minutes. When signing the IVC, 36.4% of patients asked one to three questions about the information contained in the document. 71.7% of patients would prefer to ask questions to a doctor, 27.3% - to medical workers, 1% - could use the help of other patients. The researchers noted that 36.4% of respondents asked three or more questions on IVC, which emphasizes the need to provide additional time at the doctor's office for the formation of communications. We consider it necessary to note that after the survey, 58.1% of respondents noted that they would become more familiar with information about IVC, and 53.7% of patients would introduce it to relatives and friends. Conclusions. The study made it possible to identify a number of topical issues for discussion and management decision-making in medical organizations: to use various sources of information for the population about the purpose of the IVC, to create comfortable conditions for patients when reading the document in the presence of a doctor or medical worker. The opinion of the respondents confirmed that the choice of a medical organization depends on the level of communication skills of the staff and confirms the need for measures to improve the communicative competence of all medical workers. The implementation of the communication skills of medical workers depends significantly on the time that can be used to discuss the information contained in the IVC.
