STAFFING OF PRIMARY HEALTH CARE FOR THE ADULT POPULATION OF THE RUSSIAN FEDERATION IN 2014-2022. PART 2: FOCUS ON GENERAL PRACTITIONERS

Summary:
Introduction (relevance). The second part of the study of the staffing of primary health care is devoted to the analysis of the number and provision of the population with general practitioners. The purpose is to analyze the indicators of staffing of primary health care and their dynamics for the period 2014-2022 in terms of general practitioners. Materials and methods: analysis of data on the number of individuals of general practitioners in 2014-2022, the security of the population and qualification categories according to the data presented in the statistical collections of the FSBI "TSNIIOIZ" of the Ministry of Health of the Russian Federation. The method of descriptive statistics is used. Results. Changes in the number of general practitioners in the Russian Federation as a whole during the study period were multidirectional: a decrease in 2014-2016, a sharp increase in 2017-2018 and a subsequent decrease until 2022. Accordingly, the indicator of the security of the population also changed. It is important to note that the growth of indicators was achieved mainly due to two subjects – the Belgorod region and the city of Moscow. There is a pronounced unevenness of indicators and their dynamics between the subjects of the Russian Federation. The share of general practitioners who had qualification categories decreased throughout the study period, but at a slower rate than district internists, and the share of doctors who had the highest qualification category in 2022 was higher than that of district internists. Discussion. The nature of the change in the number of general practitioners and the provision of the population as a whole in Russia can mainly be explained by the active introduction of general medical practice in the Belgorod region and the city of Moscow. When assessing the indicators and their dynamics, it is necessary to take into account that in most subjects primary medical care is provided either by district internists or general practitioners, therefore, in order to obtain a more complete description of primary medical care, a comprehensive analysis is needed taking into account data on both positions. Conclusions. In the Russian Federation as a whole, the period 2014-2022 was characterized by a decrease in the number of general practitioners by 6.6%, and the provision of their population by 7.5%. At the same time, in Russia as a whole, the nature of the changes was as follows: a decrease in 2014-2016 (-475 people), then, unlike district internists, in 2017-2018 there was a marked increase (+2270 people) and a further consistent decline until 2022 (-2445 people). The corresponding dynamics was observed in the provision of the population. The Belgorod Region had the highest rate of provision of the population with general practitioners in 2022 (2.67 per 10,000 population), while two subjects – the Republic of Ingushetia and the Chechen Republic – do not have general practitioners at all. For the most complete assessment of the personnel situation in terms of primary medical care in the subjects, it is necessary to conduct a comprehensive analysis taking into account the specifics of the organization of primary health care in the subjects of the Russian Federation. In terms of qualification categories, unlike district internists, the decline in the proportion of doctors with a qualification category occurred at a slower rate and reached 21.7% in the Russian Federation as a whole in 2022. At the same time, the share of GPS with the highest qualification category increased from 9.6% to 12.0%.
