EPIDEMIOLOGY OF COLORECTAL CANCER AMONG PATIENTS WITH POLYNEOPLASIAS IN THE KURSK REGION

Introduction. The epidemiology of colorectal cancer in patients with polyneoplasias has not been studied enough, and therefore it is impossible to solve the problem of early diagnosis in this group. The purpose of the study: to analyze the epidemiology of colorectal cancer among patients with extra-intestinal malignant neoplasias in the Kursk region from 2018 to 2022. Materials and methods. Statistical reports of the Kursk region from 2018 to 2022 are analyzed: "Information on patients with malignant neoplasias" and "Information on diseases with malignant neoplasias", logs of registration of endoscopic examinations. The study group included 218 patients who had primary multiple cancer with the participation of colorectal cancer from 2018 to 2022, the medical documentation of these patients is analyzed. Results. The majority are oncodermatological patients: 20.2% (44) cases, the second place is taken by oncourological patients – 19.7% (43), the third – by patients with tumors of the female reproductive system: 14.2% (31) cases. The overwhelming number of patients are in the group with metachronous primary multiple cancer – 75.7% (165). The relationship between age and extra-intestinal diagnosis (p = 0), localization of extra-intestinal neoplasia and variable synchronicity/metachronicity, (p = 0.006), the stage of colorectal cancer with the affected part of the intestine (p = 0.024), the department of colon lesion with the primary/secondary detection of colorectal cancer (p = 0), the variables of primary/secondary colorectal cancer and synchronicity/metachronicity (p = 0), the interval between the detection of neoplasia and the type of extra-intestinal tumor, p = 0, the stage of the detected colorectal cancer and the year of detection of colorectal cancer (p = 0.015), the stage and interval between the detection of neoplasia, p = 0.032, the interval between the diagnosis of neoplasia and primary/secondary, p = 0, are found. An increase in the detection of the first stage is found, which in 2022 became the leading 33.3% (21) of cases. Discussion. The significance of the diagnosis of colorectal cancer in the group of patients with polyneoplasias is proved: in 2022, patients of the study group accounted for 13% of all cases of primary multiple cancer detected in the Kursk region: colorectal cancer is diagnosed in 1.5% of patients registered with gynecological tumors, 1.1% - oncourological profile, 1.1% - with skin malignant neoplasias. On the other hand, patients in the study group account for 12.5% (63 out of 505) of all cases of colorectal cancer diagnosed in 2022 in the Kursk region, i.e. colorectal cancer can be considered neoplasia with a high risk of polyneoplasias. Conclusions. Thus, it is necessary to show oncological alertness with respect to the colon in the group of patients with polyneoplasias, especially in the group of patients with tumors of the skin, urological profile and female reproductive sphere.
