PECULIARITIES OF MEDICAL ASSISTANCE TO WOMEN BY GENERAL PRACTITIONERS (FAMILY PHYSICIANS) IN THE BELGOROD REGION

Summary:
Introduction. The article presents a model of general practitioner (family physician), (hereinafter referred to as GP), on the example of Belgorod region and its role in providing obstetric and gynecological care at the outpatient stage and prevention of reproductive losses. Objective To evaluate the role of a general practitioner (family physician) in providing obstetric and gynecologic care. Materials and methods. The availability of general practitioners (family physicians) was analyzed, the level of their knowledge in the provision of obstetric and gynecological care was assessed, and the reproductive health of women of fertile age was studied. Statistical data from Belgorodstat and the regional medical information and analytical center were studied. Research methods: analytical, statistical, mathematical and test methods. Results and Discussion. During the years of reorganization of the district service in the region, the number of general practitioners' offices increased from 259 in 2016 to 614 in 2019, i.e. 72.9% more offices of general practitioners. At the same time, the assessment of the initial level of knowledge in the provision of obstetric and gynecologic care, within the framework of the survey of general practitioners in the average median value scored 49.6% of correct answers. A postgraduate training program was developed to provide obstetric and gynecological care at the outpatient stage. Over 3 years, 590 physicians and 616 nurses underwent retraining. The organization of a continuous educational process to improve the competence of GPs in providing medical care to women has made it possible to change the structure of the reasons for referrals to doctors: the number of referrals due to illness has decreased by 17%. The level of active detection of diseases among women of childbearing age has increased: circulatory system - by 39%; respiratory organs - by 6.9%; diabetes mellitus - by 4.3%; and neoplasms - by 9.6%. The share of persons under dispensary observation increased by 13%, the number of emergency medical team calls decreased by 3.3%. The number of women who installed intrauterine devices increased by 6%. The use of hormonal contraceptives increased by 15.4%. Conclusion. The importance of general practitioners (family physician) in the system of primary health care is obvious, and the organization and conduct of their continuous postgraduate training in the provision of medical care to women, has increased the level of competence of doctors by 24.2%, resulting in a 30% decrease in the number of complaints about district specialists and a 40% increase in patient satisfaction with the quality of medical care.
