MODERN PROBLEMS OF PROVIDING THE POPULATION OF THE RUSSIAN FEDERATION WITH THE MAIN HUMAN RESOURCE WITH SECONDARY MEDICAL EDUCATION IN THE STATE HEALTH CARE SYSTEM

Summary:
Introduction. In the modern world, the issue of shortage of medical personnel is quite acute. One of the main problems in the personnel management system in medical institutions is the understaffing of nursing staff. It is important to understand that this problem occurs not locally, but throughout the country, which indicates the need for a more detailed consideration of this topic, since a decrease in the number of medical workers affects the quality of work of healthcare organizations, which can lead to an increase in morbidity, a decrease in life expectancy and increasing mortality among the population. Consequently, the search for possible solutions and ways out of the current situation becomes relevant. Target. Assess the personnel shortage among workers with secondary medical education in the Russian Federation and summarize published experience in eliminating personnel shortages in the healthcare sector. Materials and methods. The review was prepared by collecting scientific sources of literature published by domestic and foreign authors over the past 10 years (2002-2022), the literature search was carried out using the keywords: “nursing-level medical workers”, “provision”, “staffing”, “human resources” in healthcare”, “medical personnel”, published in peer-reviewed scientific journals, indexed in the scientometric databases WoS, Scopus, included in the list approved by the Higher Attestation Commission under the Ministry of Education and Science of Russia. The search for scientific articles was carried out in the electronic databases PubMed, Google Scholar, RISC. Data synthesis as well as the electronic search strategy were conducted according to PRISMA principles. 21 scientific articles were selected for analysis. An analysis of the official statistics of Rosstat was carried out. Results and discussion. Modern staffing problems show that, despite being well equipped with highly qualified medical equipment, trained nurses are required. The provision of health care with paramedical workers, according to Rosstat, states that in the constituent entities of the Russian Federation there is a decrease in the supply of nurses to the population. The number of medical personnel is maintained due to population decline. The influencing factors on the trend in the provision of health care with paramedical workers were demographics, the factor of professional attractiveness, territorial, and professional migration. Along with the factors, there are also reasons for the shortage of mid-level workers: aging of healthcare workers; transition of medical workers to higher paid jobs; lack of specialists coming to vacant positions; insufficient number of young people wishing to obtain or have already received the profession of a doctor; the growing global population at risk from non-communicable diseases; migration of health workers. Conclusions. When studying the issue of shortage of specialists with secondary medical education, the need was identified for reorganizing the structure of medical personnel and adopting a set of practical recommendations aimed at increasing the staff of mid-level medical staff.
