TERRITORIAL PLANNING AND REGULATION. RESULTS OF THE SURVEY OF HEALTH CARE ORGANIZERS

Relevance. The applied usefulness of surveys in medicine has been recognized by both respondents and health care specialists, and this practice becomes even more important in getting feedback, and is embedded in everyday life. By surveying health professionals, it is possible to get feedback on a variety of aspects, which makes it possible to analyze the awareness of the main provisions of the regulatory framework, to study the problems of medical workers in the field, to find out the effectiveness of a particular treatment methodology, and much more. In order to form a holistic view of the importance of territorial planning in health care, it is necessary to get feedback on how well this concept is known to health care organizers, how territorial planning schemes are developed at the level of Russian regions, what level of specialists are involved in territorial planning and, most importantly, how the developed schemes are implemented in regional practice. Purpose. Study the application of normative acts regulating the aspects of territorial planning; to analyze expert proposals to improve the territorial accessibility of the health care system at the regional level. Мethods. During this research the results of the questionnaire survey of health care organizers on the issues of territorial planning regulation were analyzed. Statistical and sociological methods were used. Results and discussion. The study provides an idea of the current level of development and application of approaches to territorial planning in health care, its practical significance and effectiveness in ensuring accessibility of medical care. In general, representatives of health care organizers consider themselves aware of territorial planning, of the regional regulatory framework governing this process. The survey participants identify three main problems in ensuring accessibility of medical care delivery at the regional level: 1) lack of transport accessibility of medical organizations; 2) the presence of categories of patients deprived of the opportunity to receive medical care or having only partial access to it; 3) widespread staff shortage in medical organizations at all levels. According to the opinion of the interviewed heads of executive authorities of the constituent entities of the Russian Federation in health care, in addition to the problem of insufficient funding for the implementation of approaches to territorial planning, a significant obstacle is the lack of involvement and feedback from the population and health workers. The study demonstrated the effectiveness of using questionnaires to measure the objective level of respondents' awareness of territorial planning in health care, their perceptions of the mechanisms, effectiveness, problems of this process and ways to solve them. Conclusions. An important factor in ensuring accessibility of medical care is the involvement of heads of medical organizations in addressing issues of territorial planning. When developing local territorial planning schemes, peculiarities of subjects of the Russian Federation unjustifiably take a back seat. They should be considered as determinant in the development of a strategy for building spatial planning schemes for each locality, taking into account the availability of medical care for the population.
