LOST YEARS OF POTENTIAL LIFE FROM ALCOHOL AND DRUG POISONING IN THE SAMARA REGION

Summary:
Introduction. The consumption of psychoactive substances by the population is a serious medical and social problem of modern societies, which negatively affects various indicators, including mortality of the population. For the socio-economic assessment of the mortality of the population from alcohol and drug poisoning, it is of interest to use the indicator "Lost years of potential life" (PGPJ). Purpose: to study the dynamics of the PPH indicator from drug and alcohol poisoning in the Samara region in 2017-2021, conducted on the basis of data on mortality rates, and forecasting the dynamics of the value of the lost years of potential life for the nearest period (up to and including 2024). Materials and methods. Data on persons who died from acute alcohol and drug poisoning in the Samara region in the period from 2017 to 2021 were obtained from the regional information system "Mortality", demographic data on the average annual sex and age composition of the population, on life expectancy in various groups were obtained from Samarastat. In order to calculate the PPG, the under-lived years for each deceased were calculated first. The lost years of potential life when using the police registers of the deceased are calculated as the sum of the under-lived years in each study group. The forecast of the dynamics of the value of PPH from alcohol and drug poisoning was made by constructing trend lines using linear approximation, with the calculation of the magnitude of the accuracy of the approximation. Statistical processing was carried out using the SPSS statistical package (version 22). Statistical and analytical research methods were used. Results and discussion. During the entire observation period (2017-2021), 831 people died from acute alcohol poisoning in the Samara region. Of all the deceased, the proportion of men was 80.5%, women – 19.5%. The average age of the deceased men was 50.0±0.5 years, women - 52.9± 0.9 years. During the same period, 520 people died from drug poisoning. The average age of the deceased was 34.9±0.3 years. Of all the deceased, the proportion of men is 87.7%, women - 12.3%. The average age of the deceased men was 35.2±0.3 years, women - 33.5± 1.0 years. Despite the higher mortality rates from acute alcohol poisoning (5.23 per 100 thousand population) compared with deaths from drug poisoning (3.27 per 100 thousand population), the total number of lost years of potential life from drug poisoning (16417.9 years) is higher than from alcohol poisoning (14168.2 years). The reason lies in the statistically significant difference in the average age of those who died from fatal alcohol poisoning (50.6 ± 0.4 years) and drugs (34.9± 0.3 years). The forecast of the PHC value in 2024 from alcohol poisoning in the Samara region with the calculation of the approximation reliability value is due to the downward trend line and may be 43.8% lower than the level of 2021. The forecast of the value of GWP in 2024 from drug poisoning in the Samara region is also due to the downward trend line and may be below the level of 2021 by 34.7%. Conclusion. The estimated value of the indicator "Lost years of potential life" is a fairly informative indicator of the socio-economic assessment of the indicator of premature mortality due to alcohol and drug poisoning. A statistically significant difference in the age of death from acute drug and alcohol poisoning in the Samara region determines the greater number of lost years of potential life from fatal drug poisoning. This situation indicates the need for further implementation of measures to reduce cases of fatal drug poisoning in the Samara region, including in the target group of drug users. Making a forecast of the dynamics of the number of GWP by constructing trend lines suggests a further decrease in the estimated values in the next three-year period.
