THE CORRELATION OF CLINICAL FORMS AND MANIFESTATIONS OF ATHEROSCLEROSIS IN ELDERLY PATIENTS

Summary:
Introduction. The clinical forms and manifestations of atherosclerosis depend of the location of atherosclerotic plaques. The common pathogenetic mechanisms of these plaques formation allow expecting a certain correlation between the clinical forms and manifestations of atherosclerosis. The study of such correlation can help to diagnose the multiple and concomitant atherosclerotic lesions. Materials and methods. It was examined 99 patients with various clinical forms and manifestations of atherosclerosis. All patients were aged 65 and over, they were examined and treated at the Vascular Surgery Department of the Belgorod Regional Clinical Hospital of St. Joasaph. Ultrasound duplex scanning and bolus contrasted spiral computed tomography were used for more objective detection of vascular lesions. The relationships were estimated between angina pectoris, postinfarction cardiosclerosis, atrial fibrillation, atherosclerosis of the lower extremities and brachiocephalic arteries, the presence of coronary artery bypass grafting and/or cerebral stroke in a history. Statistical processing of the research results was performed by using of computer technology and software package STATISTICA. Results. As a result of the study, it was found that elderly cases with coronary heart disease most often have manifestations of the brachiocephalic arteries atherosclerosis (33.33% of the cases), postinfarction cardiosclerosis (29.29% of the cases) and lower extremities arteries atherosclerosis (28.28% of the cases). A positive noticeable relationship between the brachiocephalic arteries atherosclerosis and the presence of a history of acute cerebrovascular accident has been revealed. There was also a positive moderate relationship between postinfarction atherosclerosis and coronary artery bypass grafting in a history. The detection of atherosclerosis of the brachiocephalic arteries and/or a history of brain stroke was accompanied by a decrease of the number of detected cases of the lower extremities atherosclerosis . In addition, there was a weak negative relationship between atherosclerosis of the brachiocephalic arteries and the presence of a history of coronary artery bypass grafting in the patients. Conclusion. In elderly, there are some correlations between the clinical forms and manifestations of atherosclerosis that has variable intensity and orientation. Knowledge of the relationship between the clinical forms and manifestations of atherosclerosis can help diagnose multiple and concomitant atherosclerotic vascular lesions.
