ASSOCIATION OF DEFICIENCY CONDITIONS IN PATIENTS WITH COMORBID ARTERIAL HYPERTENSION AND CORONARY HEART DISEASE

Summary:
Introduction. Arterial hypertension and coronary heart disease are common comorbid diseases that significantly change the geriatric status. However, deficiency syndromes in patients with comorbid hypertension and coronary heart disease remain poorly studied and are mainly represented in publications by anxiety and depression. The aim of the study - study the association of complex deficiency conditions in patients with comorbid arterial hypertension and coronary heart disease. Material and methods. In clinical conditions, 125 patients aged 60-74 years with comorbid arterial hypertension and coronary heart disease were examined, and 118 elderly patients suffering from isolated arterial hypertension, the most common cardiopathology, served as a comparison group. The diagnosis of arterial hypertension and coronary heart disease was carried out based on the results of a comprehensive cardiological examination and taking into account National recommendations prepared for these nosological forms. The results of the study. The greatest association of frailty with arterial comorbid hypertension and coronary heart disease was shown among elderly patients with a relative risk value in uncorrected logistic regression – OR=4,32 (p=0,0001). Associations of comorbid arterial hypertension and CHD with sleep disturbance syndrome (OR=3,18; p=0,0013) and hypomobility syndrome (OR=2,37; p=0,0014) were found to be statistically significant in uncorrected logistic regression. Associations with other deficit states were also statistically significant. The multivariate logistic regression performed with age and gender adjustment showed that all the studied deficit syndromes have statistical significance. At the same time, the leading deficiency syndromes associated with comorbid arterial hypertension and coronary heart disease in elderly patients are, as in the previously established results, the syndrome of senile asthenia, sleep disorders and hypomobility. Confidence intervals of the above-mentioned deficiency syndromes and other studied deficits are statistically significant. Conclusion. Important deficiency syndromes associated in elderly patients with comorbid arterial hypertension and coronary heart disease are the syndrome of senile asthenia, sleep disorders and hypomobility, which should be taken into account in gerontoprophylaxis and rehabilitation of this contingent.
