THE ROLE OF OBSTETRIC RISK FACTORS AND RESOURCE PROVISION OF OBSTETRIC SERVICES IN REDUCING INFANT MORTALITY

Summary:
Introduction: in the context of unfavorable demographic trends, a reserve for further combating child mortality in the first year of life may be reducing the negative impact of obstetric risk factors and increasing the availability of medical care for women during pregnancy, childbirth and the postpartum period. Purpose: to assess the impact of individual obstetric risk factors and resource provision of obstetric services on the infant mortality rate. Materials and methods: based on data from Rosstat, statistical materials from the Federal State Budgetary Institution «Central Research Institute for Organization and Informatization of Health Care» of the Ministry of Health of Russia and Form No. 32 “Information on medical care for pregnant women, women in labor and postpartum” for 2018-2022. An analysis of infant mortality rates, morbidity rates among women of fertile age according to the XV class of diseases ICD-10 - Pregnancy, childbirth and the postpartum period, the frequency of complicated births and the availability of obstetric service personnel and obstetric beds was carried out. To assess the relationship between indicators, the nonparametric Spearman rank correlation method was used. Statistical data processing was carried out using the MS Office-2016 and StatSoft-Statistica 10.0 software packages. Results and discussion: Despite the significant decrease, the mortality rate of children in the first year of life in the Chechen Republic exceeds the Russian and regional averages. Against the background of a high level of morbidity among women of fertile age in the class of diseases - Pregnancy, childbirth and the postpartum period, a low frequency of complicated births is observed in the region. The region's obstetric service is characterized by high resource availability, both medical personnel and obstetric beds. There is an annual increase in the share of pregnancy pathology beds in the structure of obstetric beds in the Chechen Republic. An assessment of the correlation between infant mortality rates and indicators of the provision of midwives, obstetrician-gynecologists and pregnancy pathology beds in the region in 2018-2022. revealed the presence of an inverse strong and moderate relationship between the indicators (rxy from -0.52 to -0.79). Accordingly, increasing the availability of obstetric care for women and increasing the share of pregnancy pathology beds in the total number of obstetric beds have a significant impact on reducing mortality rates for children in the first year of life. Conclusion: In the Chechen Republic over the past five years, there has been a significant decrease in infant mortality, which was largely due to the high availability of obstetric care for women during pregnancy, childbirth and the postpartum period. A reserve for further reducing child mortality in the first year of life is reducing the incidence of women during pregnancy through timely detection and treatment of diseases in antenatal clinics. Reduced availability of obstetric care in the Chechen Republic is a regional risk factor for increasing infant mortality rates.
