IMPACT OF THE COVID-19 PANDEMIC ON THE TUBERCULOSIS CARE SYSTEM IN THE RUSSIAN FEDERATION

Summary:
Introduction. Various aspects of the impact of the COVID-19 pandemic and related population mobility restrictions ("lockdown") on tuberculosis care have been described worldwide. Objective: to comprehensively assess the impact of the COVID-19 pandemic on the tuberculosis care system for the population of the Russian Federation. Materials and methods: we used data from federal and departmental statistical surveillance and operational monitoring. Key data describing individual components of the tuberculosis care system were compared with the expected values obtained by regression analysis of the data for the previous 5 years or their values for 2019. Results. There was a decline in TB chemoprophylaxis coverage for children in contact with TB patients and people living with HIV, which did not lead to an increase in their TB incidence. The lockdown in the early months of the pandemic led to a decline in TB screening coverage and a subsequent recovery, more so in children. This led to an increase in passive and post-mortem case detection, a drop in the number of new cases and a decline in the number of recurrent TB cases (by 11.6% and 6.3% of the expected number, respectively), especially pronounced among foreign nationals and homeless people. The number of newly detected TB cases was decreasing due to both lockdown and overloading of the health system. The proportion of newly detected TB cases with sputum smear-positive TB and destruction of lung tissue has increased. Compared to 2019, the proportion of patients not examined by bacteriological methods and drug susceptibility tests has slightly increased (from 2.5% to 2.9%). The impact of COVID-19 on the increase in the proportion of the newly detected patients who interrupted their chemotherapy course (from 5.6% to 6.2%) cannot be excluded. There was no deterioration in drug availability compared to 2019. There has been a moderate (12.4 - 15.1%) decrease in referrals for hospital admission to TB hospitals for children and adults and a significant (over 40%) decrease in referrals to sanatoriums. There was no increase in mortality from tuberculosis, but there was an increase in the number of patients, detected post-mortem, and in the number of newly detected children with residual changes after tuberculosis. Conclusion. The COVID-19 pandemic and its associated lockdown had the greatest impact on active TB case detection and chemoprophylaxis. There was a decrease in the number of TB patients (newly detected and recurrent), with an increase in the proportion of sputum smear-positive patients and patients with lung tissue destruction, the proportion and number of post-mortem cases and newly detected children with residual changes after tuberculosis. The impact of the pandemic on the rest of the TB care system in Russia is insignificant or doubtful.
