EFFECT OF BCG / BCG-M VACCINATION IN CHILDREN 15-17 YEARS OLD

Summary:
Introduction. The protective effect of mixed BCG / BCG-M vaccination in children 7-14 years old has not been studied, although this has practical value. Aim: to study the protective effect of BCG / BCG-M vaccination in the Russian Federation in children aged 7-14 years under conditions of mixed immunization with BCG / BCG-M vaccines. Methods: a retrospective case-control study of 3500 children aged 7-14 years with new cases of tuberculosis or residual changes after it, registered in 2018-2020, including 1578 patients with tuberculosis with or without bacterial excretion, 1573 patients with localized and generalized tuberculosis, 455 pulmonary TB patients. The protective effect was assessed according to the following criteria: the ratio between active tuberculosis and residual changes after suffering tuberculosis, the ratio between localized and generalized TB, the proportion of the frequency of bacterial excretion (by any method), and the proportion of the frequency of cavities in pulmonary TB patients. Results. Immunization with BCG / BCG-M shifted the relationship between tuberculosis and residual changes after it; at the same time, the influence of age, method of detection and contact with a patient with tuberculosis was excluded (aOR = 0.7; p = 0.04). There was no statistically significant change in the ratio between localized and generalized tuberculosis (OR = 1.2; p = 0.7), tuberculosis with and without bacterial excretion (aOR = 0.8; p = 0.6), pulmonary tuberculosis with cavities and without them (aOR = 0.5; p = 0.3). Conclusion. The protective effect of the vaccine against tuberculosis in conditions of mixed immunization with BCG / BCG-M vaccines in children 7-14 years old is questionable. The results of the study justify an additional study to clarify the protective activity of the BCG and BCG-M vaccines.
