DYNAMICS OF INDICATORS OF HOSPITAL LETHALITY

Summary:
In the conditions of COVID-19 pandemic the organization of work of hospitals and work of health workers has to be aimed, first of all, at increase in availability and quality of medical care. All-hospital lethality - one of the main criteria of quality of medical care. The purpose - the comparative analysis of all-hospital lethality from infectious diseases in the Russian Federation during 2019-2020. Materials and methods. Research unit – the territorial subject of the Russian Federation. In the course of work the population including all territorial subjects of the Russian Federation (85) was considered. Information sources - a form of state statistical reporting (SSR) No. 14 "Data on activity of the divisions of the medical organization providing medical care in stationary conditions" (the order of Rosstat on the statement of a form of 27.11.2015 No. 59, of 27.12. 2016 No. 866). For assessment of the obtained data simple (unweighted) arithmetic-mean sizes, value of a mean square (standard) deviation from average sizes, coefficient of variation (the relation of a mean square deviation to average size) were used. The set is considered uniform if coefficient of variation does not exceed 33%. Results. In total in the Russian Federation in 2019 in the round-the-clock hospitals 29554.3 thousand people, and in 2020 – 24846.8 thousand people were hospitalized. 582.1 thousand and 780.9 thousand people in 2019 and 2020 died according to. Thus, on average in the country on all profiles of beds all-hospital lethality increased from 1.97% to 3.14%. In absolute figures in 2020 against the background of decrease in comparison with previous year of volumes of the hospital help for 16% or for 4707.5 thousand cases of hospitalization, in hospitals died almost on 200.0 thousand people more, than in 2019. In 2019 with the diagnoses belonging to the class "Infectious diseases" (codes on MKB-10 from A00 on B99) in the round-the-clock hospitals of the country 1288.1 thousand people, in 2020, without patients with a new coronavirus infection were hospitalized, 754.7 thousand people are hospitalized, i.e. is 30% less. All-hospital lethality at infectious diseases on average in the Russian Federation in 2020 in comparison with 2019 increased by 1.5 times – from 1.88±1.00% in 2019 to 2.85±1.44% in 2020. Specific weight of number of the persons who died in hospitals at working-age was 84% in 2019 and 83% in 2020. Most often at working-age in the round-the-clock hospitals of an infectious profile patients with HIV (96%) die. For treatment of COVID-19 in 2020 181298 beds were allocated (174830 for adults and 6468 for children). At the same time 63428 specialized beds were in addition developed in 69 territorial subjects of the Russian Federation, most of all in the Moscow region – 8424 beds and in Moscow – 8117 beds. During 2020 in the conditions of the round-the-clock hospital 2.0 million patients infected with COVID-19 are treated, 115.5 thousand people died. Level of all-hospital lethality at COVID-19 on average in the Russian Federation was 5.6%±2.9 (coefficient of variation – 51.7%). Conclusions. Growth of level of all-hospital lethality at infectious diseases, high variability of the considered indicator in a section of territorial subjects of the Russian Federation confirm deterioration of medical care. Target federal and regional programs for strengthening of infrastructure of regional infectious services and also strengthening of coordination of activity of these services from profile federal chief specialists and also head profile federal scientific centers are necessary.
