PROJECTS TO IMPROVE PRIMARY HEALTH CARE IN MOSCOW, AS A BASIS FOR CHOOSING SCIENTIFIC DIRECTIONS OF PRIMARY CARE SPECIALISTS

Summary:
Introduction (relevance): the introduction of modern methods of organizing medical care is a priority task, the solution of which will reduce mortality rates, staff turnover, and increase the efficiency of labor in the field of health care. At the same time, the introduction of the research component of the organization of healthcare and public health in the outpatient department faces certain difficulties that must be overcome in order to improve the efficiency of the entire system. Purpose: the development of the healthcare system, including primary health care, can be a catalyst for the economic growth of cities through attracting direct investment, creating additional interest by increasing the demand for medical tourism, as well as creating incentives for the development of related sectors of the economy: pharmaceutical and medical industries, construction sectors and areas of scientific research. Materials and methods: In cities with the most effective systems of primary health care, a fundamental restructuring of the organization of medical care is taking place. One of the most important organizational trends is the transition from models organized around medical institutions and individual doctors to patient-oriented models built around the needs of the patient and involving integrated management of human health at all stages of care. Results: To date, a number of serious problems in the organization of PHC have been resolved. First of all, queues to doctors have been drastically reduced and the availability of diagnostic tests has increased. Currently, more than 80% of patients can get an appointment with a local doctor on the day of treatment (in 2010 - 32%), in the first 3 days - 96%. By 2024, 200 facilities will be overhauled, which were allocated (selected) on the basis of a survey by a specialized organization, taking into account the technical condition and needs, as well as the opinion of the population served. Discussions: The health care system today is being reoriented towards more effective and cost-effective disease prevention and timely diagnosis to detect diseases at an early stage. Such preventive measures allow for faster and less costly treatment, the cost of which steadily increases as the disease progresses or becomes chronic. Conclusions (conclusion): the role of research activities at all levels of the health care system cannot be overestimated. Primary care specialists should have the necessary and relevant knowledge about the development of medical science in the world, the most popular scientific achievements, the main trends in the development of primary health care, new knowledge about the patterns of development of personalized medicine, improving the quality of medical care, sustainable scientific and technological, social - economic development, strengthening the health of citizens.
