PROTECTION OF THE POPULATION REPRODUCTIVE HEALTH: ANALYSIS OF THE PRENATAL SCREENINGS’ RESULTS IN THE KURSK REGION

Summary:
Introduction. Prenatal screening is aimed at forming a risk group for hereditary and congenital pathology of the fetus. According to the World Health Organization, more than 270 thousand newborns die every year from congenital malformations during the first month of life, and more than 3 million surviving children have disability of varying severity. Currently, prenatal screenings are widespread, that is, they are recommended for all pregnant women, regardless of family history and the course of pregnancy, which ensures the protection of the population reproductive health. Aim. To conduct a comparative analysis of the prenatal screenings’ results to identify pregnant women at risk of developing fetal pathologies and chromosomal abnormalities in each of the districts of the Kursk region. Material and methods. Based on the data of the obstetric and gynecological database Astraia of the medical genetic consultation of the Kursk region, using the software, the individual risks of having a child with a chromosomal abnormality, growth retardation, preterm birth and preeclampsia were calculated. Share of women of childbearing age who underwent prenatal screening, share of pregnant women at risk based on the results of screening, share of women at risk who refused invasive diagnostics in the Kursk region as a whole and in each individual district in 2021 were characterized. Results and discussion. In the Kursk region in 2021, the proportion of fertile age women who underwent prenatal screening averages 3.07%. The actual rate of coverage of pregnant women who underwent prenatal screening in the first trimester was 83.3%. In 19 districts of the Kursk region out of 28, the proportion of women who underwent screening was below the average for the region. In Kursk, Zheleznogorsk and Kurchatov, this figure was above the established average. In 7 districts of the Kursk region, not a single pregnant woman was classified as a risk group for fetal chromosomal abnormality. Of the 153 pregnant women who were at risk for fetal chromosomal abnormality, 37.9% refused to undergo an invasive study. Conclusion. To improve the indicators of prenatal screening in the Kursk region, it is necessary to carry out educational work among the population and ensure timely referral of pregnant women for examinations to obstetric medical organizations of the second or third level. The proportion of pregnant women classified as a general risk group and a risk group for chromosomal abnormalities depends on a number of factors, in particular, on the characteristics of the organization of health care and the ecological characteristics of the areas. For municipalities with high values of this indicator, it is necessary to analyze the data in dynamics and identify factors that affect the reproductive health of the population.
