Summary:
Relevance. As you know, opportunities and achievements in medicine directly depend on the degree of development of other branches of knowledge. Thus, thanks to the rapid development of materials science, production technologies, engineering, it has become possible to significantly increase the number of high-tech types of surgical interventions in traumatology and orthopedics, in particular endoprosthetics of large joints. However, the rapid replication of the method and the imperfection of implantable structures, especially critical for young patients with a high degree of activity, led to a sharp increase in the number of revision operations and the appearance of severe bone changes that make it difficult or in some cases impossible to install a standard implant. In these situations, the "ideal" solution at the current level of technology development is individual implants made by additive technologies, which can be considered with a high degree of optimism as a new promising "option" for severe bone defects and/or highly biologically compromised bone tissue. The purpose of the study. increasing the cost-effectiveness of joint re-arthroplasty operations to improve medical technologies and the introduction of new treatment methods, in particular, the use of personalized implants at the expense of the CHI and VMP fund. Material and methods. In the clinic of the R.R. Vreden T&O Center, together with bioengineers of various companies, 404 individual structures were developed and implanted during primary and revision hip replacement to replace defects not only of the acetabulum, but also of the proximal femur, as well as the condyles of the femur and tibia during the revision of the knee joint, when it is impossible to install serial designs. The medium-term results of revision arthroplasty using individual structures have shown high efficiency and thus confirm the need to use these structures in terms of reducing the risk of various complications and long-term positive functional and radiological postoperative results. At the same time, it will be possible to achieve savings of up to 25-30% of the funds spent on providing high–tech assistance of the appropriate profile by leveling the development of many complications and increasing the survival time of implants. Results. a new type of high-tech care is calculated, based on the experience of implementing technologies in the process of clinical testing and the needs of patients in the presented technologies, which allow for a personalized approach to implant various joints after the progression of periprosthetic infection.
