NEW APPROACHES IN SCHOOL MEDICINE DURING MEDICAL AND PROFESSIONAL COUNSELING OF ADOLESCENTS

Summary:
Introduction. Currently, the system of medical support for adolescents in the field of medical and professional counseling requires improvement. There is a problem of employment of a graduate specialist with professional selection, as well as rapid disability under the influence of unfavorable production factors. Taking into account the modern achievements of personalized medicine, it becomes obvious the benefits of using genetic markers of adolescents as their predicted professional reliability in the future profession during medical and professional counseling.
The purpose is to characterize the molecular genetic characteristics of adolescents who determine stress resistance as a criterion of professional reliability during medical and professional counseling.
Materials and methods. A single-stage study was conducted among 198 adolescents aged 14-15 years, in whom genetic biomarkers of phenotypic features of the level of psychophysiological functions and behavior were studied. For this purpose, a comparative assessment of the polymorphism of COMT genes (rs 4680, Val158Met (C472A) COMT), the CPOX gene, and the MHRF gene in adolescents with varying severity of stress levels was carried out. Phenotypic manifestations of the level of psychophysiological functions and behavior were studied by the level of anxiety, negative emotional experiences (Spielberger Ch.D. in the adaptation of A.D. Andreeva, 1988), as well as the severity of stress resistance (Rybnikov M.Yu., 1985). The evaluation of the significance of differences in the studied indicators of mental health between groups of adolescents with different levels of stress was carried out by calculating the Pearson criterion χ2.
Results. It was found that among adolescents aged 15-17, a high level of stress resistance was determined in 53.3% of individuals. In adolescents with a high level of stress, compared with adolescents who do not have a high level of stress, persons with a high level of anxiety are more common by 2.91 times (OR 2.91; CI = 0.52-1.6; χ2=15.96; p=0.01); with a high level of negative emotional experiences by 1.6 times (OR 1.6; CI = 0.15-0.88; χ2=7.99; p=0.01).In the group of carriers of the "pathological" allele in the COMT, MTHFR and SROX genes, in contrast to the "normal" homozygotes in the same genes, a more pronounced deterioration of mental health was revealed by mental factors of the current state (MannWhitney U=449,000, p
