QUALITY ASSESSMENT OF THE SCREENING TEST FOR PREDICTORS OF ACUTE CORONARY SYNDROME

Summary:
Introduction. The problem of acute coronary syndrome is one of the important problems in the field of railway medicine, since the safety of the transportation process depends on the state of health of the employee who provides it.
Aim. To assess the quality of the screening test for predictors of acute coronary syndrome using statistical quality control of the verified diagnostic test (with binary outcomes).
Materials and methods. In 2008-2013 period, a follow-up of 22 items of the group of initially healthy 7,959 men (employees of locomotive crews of the Trans-Baikal Railway) aged 18-66 years was conducted. As a result of the observation such predictors of acute coronary syndrome as excessive alcohol consumption, arterial hypertension, I-II degree retinopathy, left ventricular myocardial hypertrophy, psychosocial stress, type 2 diabetes mellitus, and age 36-66 years were established. DiagStat was applied to evaluate their predictive power when used in screening tests for acute coronary syndrome.
Results. Diagnosing excessive alcohol consumption which suggests systematic daily single alcohol consumption above the WHO safe limit, i.e. more than 2 standard doses of alcohol per day with 1 dose equal to 13.7 g [18 ml of ethanol] in terms of alcoholic beverages, moderately increases the posttest probability of the occurrence of an acute coronary syndrome versus its absence, as compared with the prior probability after receiving a positive result of a diagnostic test being verified in a patient.
Conclusion. It is necessary to conduct a targeted search for a predictor of excessive alcohol consumption through a diagnostic algorithm for chronic alcohol intoxication since the detection of this predictor of acute coronary syndrome in workers of locomotive crews is not stipulated by the existing regulations. Other predictors of acute coronary syndrome also need to be diagnosed in order to subsequently correct them in the patient.
