USING THE POPULATION BIRTH REGISTER FOR ESTIMATION OF PREVALENCE OF SEXUALLY TRANSMITTED INFECTIONS IN PREGNANT WOMEN

Summary:
Introduction. Screening pregnant women for sexually transmitted infections (STIs) is an important part of the antenatal care. The prevalence of these diseases is monitored at the population level using federal statistical surveillance forms, but they contain only aggregate data. One of the promising methods for such monitoring is population-based birth registries, that constitute important part of e-health in many countries.
Aim. To evaluate the possibility of using the Arkhangelsk County Birth Registry as a tool to monitor the prevalence of STIs in pregnant women and to investigate socio-demographic factors associated with this group of diseases.
Materials. A cross-sectional study was conducted based on data from the Arkhangelsk County Birth Registry (RRAO), which contains data on 93,000 pregnancy outcomes with gestational age of 22 weeks or more in the Arkhangelsk region. We used data for 2014-2018 for the current study. Number of pregnancy outcomes included in the analysis was 63728. The prevalence of STIs among pregnant women in the Arkhangelsk region in 2014-2018 was calculated and temporal trends were assessed. The prevalence in different groups were compared by Pearson's chi-square test.
Results. The reported prevalence of all STIs among pregnant women in the Arkhangelsk region during the study period was 21.9 cases per 1000 women (95%CI: 20,8-23,1). Higher prevalence of syphilis was registered in the group of pregnant women over 35 years old. Comparing the prevalence depending on marital status, the highest prevalence of syphilis and other STIs except for gonococcus infection was registered in the group of single women. A significant upward trend in the prevalence of all STIs was detected with a decrease of educational level.
Conclusions. The Birth Registry is an important tool for monitoring the morbidity of pregnant women, which allows to be used to monitor STIs in this group. The prevalence of STIs among pregnant women in the Arkhangelsk region in 2014-2018, estimated using the birth register, was 21.9 per 1,000 cases. A woman's age, marital status and level of education were significantly associated with this indicator.
