SOME ASPECTS OF OVERCOMING THE PERSONNEL CRISIS IN PRIMARY HEALTH CARE

Summary:
Introduction. The aggravation of the political situation in the world, the emergence and spread of new pandemics of infectious diseases in modern conditions predetermine the growth of the negative impact of the risks of the functioning of the healthcare system: demographic, personnel, financial, managerial and others. One of the significant risks of successful management of the healthcare system is the problem of the quality of professional training of healthcare organizers of the management group. The large area of Russian territories, low population density, the concentration of the technological base, material, and human resources in the healthcare of large cities contribute to the development of negative trends in the organization of medical care for the rural population. The development of a personnel crisis leads to risks in the healthcare system, contributing to limited availability and a decrease in the quality of medical care for the population.
Goal. To study the opinion of medical specialists providing medical care to patients with diabetes mellitus in outpatient medical organizations of the Saratov region on ways to solve the problem of insufficient staffing.
Materials and methods. Original questionnaires were developed as a material: a questionnaire for doctors providing primary health care, as well as a questionnaire for experts - endocrinologists. Analytical and statistical research methods were used. The definition of representative sample populations was carried out. The significance of the difference between independent samples was assessed using the Student's t-test, in which the difference in relative values was considered as statistically significant at t ≥ 2, p ≤ 0,05.
Results. When asked about the advisability of partially delegating the rights to provide medical care to patients with diabetes mellitus to general practitioners at the outpatient stage in the districts of the Saratov region in the absence of endocrinologists in medical organizations, 95% of the surveyed primary health care physicians expressed their disagreement with this proposal. The main reasons for the negative opinion were: an increase in the workload of general practitioners (66% ± 2.1%) and a shortage of rates for general practitioners (30% ± 2%). 70% of experts - endocrinologists agree with the expediency of transferring part of the powers to district therapists and general practitioners (p
