DYNAMICS OF INDICATORS OF ACTIVITY OF MEDICAL ORGANIZATIONS OF MOSCOW PROVIDING ASSISTANCE IN INPATIENT CONDITIONS

Summary:
Introduction. The structural and functional transformations taking place in Russian healthcare since 2006, aimed at reducing the number of round-the-clock beds, eliminating inefficient medical organizations, have led to a decrease in the provision of beds to the population, a decrease in the availability of specialized medical care in some subjects of the Russian Federation. Objective: to study the dynamics of performance indicators of medical organizations in Moscow providing care in inpatient settings around the clock in comparison with other subjects of the Russian Federation and the average in the Russian Federation for 2016-2020. Materials and methods of research: the data of the Federal statistical observation form No. 30 "Information about a medical organization" (hereinafter referred to as F. No. 30) for 2016-2020 on the activities of medical organizations providing assistance in inpatient conditions with round–the-clock stay in the Russian Federation, the Central Federal District and Moscow were studied. Statistical processing was performed using Microsoft Excel 2016 spreadsheets and the PSPP statistical data processing program. Results. In Moscow, as well as in the whole of the Russian Federation and the Central Federal District, there is a tendency to decrease the provision of the population with round-the-clock beds. The provision of round-the-clock beds in Moscow in medical organizations subordinate to the Ministry of Health of the Russian Federation decreased by 5.9%) Against the background of a decrease in the provision of beds to the population, there was a tendency to decrease the level of hospitalization over the same period. The rate of decrease in the level of hospitalization was 2-4 times higher than the rate of decrease in the provision of beds. In general, during the observation period, the level of hospitalization in Moscow decreased by 13.2%. Against the background of a decrease in the level of hospitalization, the average length of stay in bed also decreased, which is in principle justified against the background of the introduction of the latest diagnostic and treatment technologies. Therefore, with a reduction in the average length of stay on the bed, the bed occupancy rate will decrease. The rate of bed turnover against the background of a decrease in the level of hospitalization naturally decreased during the observation period. The ratio of the indicators of the level of hospitalization per 10 thousand population and the provision of round-the-clock beds, characterizing the efficiency of the use of the bed fund, also decreased after 2019 with a change in dynamics to the opposite after 2020. If we consider the above indicators characterizing the activities of round-the-clock hospitals in the context of the main profiles of beds, the picture is almost similar to all beds in general, with the only difference that in 2020 there was no significant change in general trends. Discussion. The general tendency, characteristic of most subjects of the Russian Federation, to reduce the provision of round-the-clock beds is due to the processes of restructuring the network of medical organizations with a reduction in the number and capacity of hospitals, while the level of the indicator varies significantly in the subjects of the Russian Federation. Nevertheless, the calculation and analysis of the ratio of the level of hospitalization and the provision of beds showed that Moscow was among the subjects of the Russian Federation with the highest coefficient reflecting the efficiency of the use of the bed fund. The analysis of the performance indicators of individual bed profiles led to the conclusion that the structural transformations of specialized care, including in Moscow, are unbalanced. Conclusion. The general tendency, characteristic of most subjects of the Russian Federation, to reduce the provision of round-the-clock beds is due to the processes of restructuring the network of medical organizations with a reduction in the number and capacity of hospitals, while the level of the indicator varies significantly in the subjects of the Russian Federation. In Moscow, this indicator turned out to be one of the lowest, most likely, this is due to the presence in Moscow of a large number of departmental medical organizations that take on significant amounts of medical care provided in inpatient conditions. Also in Moscow there are a large number of private medical organizations providing medical care in inpatient conditions.
