THE EFFECT OF ACCELERATED REHABILITATION ON FUNCTIONALITY AFTER SPRAINS IN ELDERLY PATIENTS

Summary:
Actuality: Ankle sprain in the elderly is one of the most common injuries of the musculoskeletal system. It is estimated that about 302,000 people are admitted to emergency departments each year in million-plus cities around the world. In the acute phase, ankle sprains are associated with pain and loss of function, and a quarter of injured people lose independence in daily life for more than seven days. Long-term risks include repeated injuries or residual problems. The preparation of accelerated functional rehabilitation may be the currently best method of treatment after ankle sprain in elderly patients, which determined the direction of our study. Objective: To determine the effect of accelerated rehabilitation on function after sprain in elderly patients. Materials and methods: We conducted a randomized controlled trial. The study included 85 patients aged 65-75 years (average age 69.8±1.4 years) who applied to the Department of Traumatology if they had an acute (less than 7 days) ankle sprain of the 1st or 2nd degree. In week 1, the main group was engaged in therapeutic exercises adapted for accelerated rehabilitation of function after sprains in elderly patients (training on a swinging board and a training program for strength and balance). While the control group underwent rehabilitation as standard. Results: The system of accelerated rehabilitation for function after sprains in elderly patients gave positive results and improved joint function indicators by 31.4% from 1 week according to the functional scale of the lower extremities. After 1 week, the main group that underwent a system of accelerated rehabilitation for function after sprains in elderly patients was significantly more active, as measured by the time spent walking (P=0.056), the average number of steps taken daily (P=0.108), and the time spent on light intensity classes (P=0.034). Conclusions: The system of accelerated rehabilitation for function after sprains in elderly patients, including therapeutic exercises during the first week after ankle sprain, led to a significant improvement in short-term ankle function compared with standard treatment.
