OVERVIEW OF THE EPIDEMIC SITUATION OF TUBERCULOSIS AMONG PERSONS HELD IN PENITENTIARY INSTITUTIONS IN 2022 (ACCORDING TO THE FORMS OF DEPARTMENTAL STATISTICAL OBSERVATION)

Summary:
Introduction. Despite the fact that the situation with tuberculosis in penitentiary institutions of the Russian Federation is assessed as having a tendency to stabilize, this disease still remains one of the serious problems of modern society and penitentiary medicine. The incidence rate
of tuberculosis in the institutions of the penitentiary system is significantly higher than the same indicator in the Russian Federation. Assessment of the epidemic situation on tuberculosis
in institutions of the Federal Penitentiary Service of Russia, based on the analysis of statistical information, it seems to be an urgent task, since it allows us to identify promising areas
for the prevention of tuberculosis and the improvement of phthisiological care.
The purpose of the study is to review the epidemic situation of tuberculosis as one of the socially significant infectious diseases, among persons detained in institutions of the penitentiary system
of the Russian Federation (UIS).
Materials and methods. The data of the forms of departmental statistical reporting of the Federal Penitentiary Service-6, Tub-4, reflecting the epidemic situation of tuberculosis in institutions
of the penal system as of 2022, are analyzed.
Results. It is established that in the period from 2015 to 2022 the situation of tuberculosis in places of deprivation of liberty, it is improving, as evidenced by a decrease in the incidence rate by 61.9%, prevalence by 43.2% and mortality from tuberculosis of suspects, accused and convicted by 84.6%. However, in 2022, there was a slight increase in epidemiological indicators for tuberculosis compared to 2021. Thus, the incidence of tuberculosis in general in the CIS increased by 0.9% compared to 2021, the prevalence rate by 1.1%. The death rate increased from 4.9 in 2021, to 5.8 per 100 thousand people in 2022, the rate of reduction in the incidence of tuberculosis in both
the pre-trial detention center and the IU was significantly lower than in previous periods
and synchronized for the first time, amounting to 3.1%.
Conclusions. In 2022, the issues of combined HIV infection and tuberculosis and multidrug-resistant tuberculosis among persons held in penal institutions were a serious problem
for penitentiary medicine. The highest level of TB/HIV from the total number of tuberculosis patients over the past 7 years was observed in 2018 - 52.3%. The use of antiretroviral therapy allowed to stabilize the share (%) of TB/HIV at the level of 32.6-35.1% of the total number
of tuberculosis patients and to achieve a 1.8-fold reduction in the incidence rate from 2015 to 2022. In 2022, there was a decrease in the incidence of TB/HIV by 7.4% and a decrease in the number
of newly diagnosed tuberculosis patients in combination with HIV infection by 12.6%. Tuberculosis with multiple drug resistance remains an urgent issue for the penal system.
This situation creates the likelihood of the prerequisites for a more serious problem of HIV-associated tuberculosis with multidrug resistance.
