PERMANENT DISABILITY IN ADULT PATIENTS WITH MODERATE AND SEVERE PSORIASIS IN THE RUSSIAN FEDERATION (ACCORDING TO PSORIASIS PATIENT REGISTRY OF THE RUSSIAN SOCIETY OF DERMATOVENEROLOGISTS AND COSMETOLOGISTS)

Summary:
Introduction: permanent disability aspects of psoriasis are still insufficiently studied. Objective: to study disability indicators, timing of primary disability among Russian patients with moderate and severe psoriasis. Materials and methods: the database of psoriasis patient registry of the Russian Society of Dermatovenereologists and Cosmetologists was analyzed. Absolute numbers of disability in adult patients with moderate and severe psoriasis were analyzed, extensive rates of disability were also calculated and analyzed. The χ2 test was used to compare categorical variables. Measures of central tendency and dispersion were used to characterize continuous quantitative variables (time till new permanent disability). Results and discussions: 13% of adult patients with moderate and severe psoriasis had permanent disability (regardless of its cause): 4% of patients with psoriasis without psoriatic arthritis, and 36% of patients with psoriasis and psoriatic arthritis. The majority (64%) of disabled patients have third-degree disability. More than a half (63%) of disabled patients had disability due to psoriasis or psoriatic arthritis. The proportion of people of working age among patients with disabilities due to psoriasis/psoriatic arthritis was 76%. In most patients, primary disability was established at working age. The median [interquartile range] period from psoriasis onset till primary disability establishment was 14 [7-22] years, the median [IQR] period from psoriatic arthritis onset till primary disability establishment was 4 [1-8] years. The results of the periods till primary disability establishment indicate rapid disablement in the majority of patients after psoriatic arthritis onset. Conclusions: psoriatic arthritis is the main cause of permanent disability among Russian patients with psoriasis. People of working age predominate in the structure of disability due to psoriasis/psoriatic arthritis. Early detection of psoriatic arthritis in patients with psoriasis and timely therapy initiation will improve clinical, as well as disability outcomes in patients with psoriasis.
