Introduction. Currently, in the context of a steady decline in infant mortality rates in Russia as a whole, in certain regions of the Russian Federation the value of infant mortality rates is heterogeneous, which determines the relevance of addressing the issue of preventing the death of children under 1 year of age. The Republic of Ingushetia belongs to regions with a high level of infant mortality (above 5.5‰). At the same time, despite the presence of a wide range of studies regarding regional features of the dynamics of infant mortality, improving prenatal diagnosis and introducing perinatal consultation in case of high obstetric and perinatal risk, in identifying congenital malformations and chromosomal abnormalities in the fetus, developing measures for the prevention of perinatal pathology, syndrome sudden death and child abuse, there is no research on the development of organizational measures to prevent infant mortality for regions with high mortality rates. Aim. Using the example of the Republic of Ingushetia, to substantiate the organizational principles of monitoring, comprehensive analysis and prevention of infant mortality for regions with a high level of mortality. Materials and research methods. The study was carried out in 3 stages. At the first stage, a content analysis of regulatory documents relating to the organization of measures regarding the organization of prevention of mortality of children under 1 year of age was carried out. At the second stage, a qualitative SWOT analysis of the organization of medical care for children under 1 year of age was performed, which made it possible to build a sequence of steps to solve problems related to improving medical care for children under 1 year of age. At the third stage, the effectiveness indicators of the implementation of the developed measures to reduce infant mortality in the Republic of Ingushetia were analyzed. Results and discussion. Based on the results of the study, the organizational principles of monitoring, comprehensive analysis and prevention of infant mortality for the Republic of Ingushetia were scientifically substantiated. The proposed system of measures includes a number of regulatory legal decisions, organization of monitoring of pregnant women and children of this age, timely provision of medical care to them, as well as measures aimed at preventing infant mortality from congenital anomalies, controllable and socially determined causes. Particular attention is paid to the interdepartmental interaction of medical organizations with bodies and institutions of the neglect prevention system to monitor families in difficult life situations or socially dangerous situations Conclusion. The implementation of developed measures to reduce infant mortality in the Republic of Ingushetia made it possible to significantly reduce infant mortality rates (below indicator indicators) by reducing deaths in the neonatal period, outside medical organizations and from certain conditions of the perinatal period. The results obtained during the implementation of these organizational principles allow us to recommend their use in other regions of the Russian Federation that have high infant mortality rates
