IMPLEMENTATION OF BRIGADE FORMS OF LABOR ORGANIZATION. JUSTIFICATION AND RISKS

Summary:
Introduction. The efficiency of medical personnel, along with providing medical organizations with qualified personnel, the active introduction of digital technologies in healthcare, and improving the material and technical equipment of medical organizations, affects the performance of the entire healthcare system. Therefore, the challenges facing health care organizers at all levels today dictate the need to revise organizational technologies for providing medical care. The purpose of the study – analysis of organizational technologies for the provision of primary health care aimed at ensuring accessibility and quality of medical care. Material and methods. A survey was conducted of the heads of medical organizations subordinate to the executive authorities of the constituent entities of the Russian Federation in the field of health care, which implemented organizational decisions to increase the accessibility and quality of medical care. Information on the implementation of such organizational decisions was provided by 469 medical organizations from 49 constituent entities of the Russian Federation. The analysis was carried out on organizational decisions implemented mainly in the provision of primary health care. Content analysis and descriptive statistics methods were used. Results. According to the survey, it was revealed that the type of medical care in the provision of which the most widespread implementation of new organizational technologies is primary health care (PHC) - 94.4% of medical organizations from among those that implemented organizational solutions (443 medical organizations out of 469). At the same time, models involving paramedics and the formation of teams in the provision of primary health care are quite common. The choice of organizational solution depends both on the specific conditions for the provision of medical care in a medical organization, and on the conditions for the provision of medical care in a constituent entity of the Russian Federation. Heads of medical organizations note the positive results of the measures being implemented. Discussions. The ongoing shortage and imbalance of human resources in the healthcare system determines the search by the heads of medical organizations and structural divisions of medical organizations for solutions that make it possible to ensure the availability and quality of medical care. The involvement of specialists with different levels of education and the redistribution of functions when performing various works corresponds to the brigade form of labor organization. The composition and number of such teams can be adapted to the specific conditions of medical care, however, in the presented organizational solutions there are no common approaches and no common understanding of the possibilities of redistributing the functions of medical personnel. Primary health care, as the main and most widespread type of medical care, includes a wide range of activities in which doctors, nursing staff and specialists without medical education can be involved. Conclusion. The brigade form of labor organization is of interest for further study, especially in the provision of primary health care. To develop and implement organizational solutions based on the redistribution or expansion of the functions of medical personnel, it is necessary to form a unified conceptual apparatus, as well as further detailed study and scientific justification of the proposed solutions, taking into account risks of their implementation.
