FACTORS AFFECTING OLDER PATIENTS' TRUST IN THE HEALTH CARE SYSTEM IN RUSSIA

Summary:
Introduction. For healthcare systems around the world, the dynamics of an aging population, rising life expectancy, and a constant increase in the number of senior people with several chronic illnesses are becoming major challenges. People over 60 in the Russian Federation have a reduced degree of trust in physicians; in light of this, study into the variables influencing trust in this age group is necessary in order to find prospects for its restoration. Purpose of the study. To research the main variables influencing Russian Federation patients who are above the working age's degree of confidence toward medical professionals, as well as the impact of trust on reasonable medical practice. Material and methods. The present work used the method of questionnaire survey with the use of a questionnaire consisting of three blocks and 89 questions. Statistical analysis of data was performed using Statistica for Windows version 10.0, Stata and R-studio programs. Interpretation of the results of machine learning models was performed using SHAP Values (SHapley Additive exPlanations. K-means method was used to perform cluster analysis. Results of the study. 3,942 persons in all took part in the poll. When asked if they entirely and primarily trusted the district general practitioner and the medical staff of the medical organization, the majority of respondents (92%, n = 3,613) said they did (n = 3,652). The study's findings include a description of the peculiarities of older adults' trust in medical professionals, as well as an analysis of the factors (such as gender, age, place of residence, communication barriers, and medical professionals' attitudes) that affect this level of trust and their relationship to age, medical activity, frequency of seeking medical attention, and the impression of a formal doctor's approach at the reception. Discussion. The outcomes of this study are similar to those of several other research carried out globally and in the Russian Federation. Conclusion. To promote sensible medical action in this demographic, greater trustworthy relationships between medical professionals and senior patients are required.
