INFLUENCE OF ADVERSE DRUG REACTIONS ON THE DEVELOPMENT OF AN ACUTE ATTACK OF ANGLE-CLOSED GLAUCOMA. A LITERATURE REVIEW

Summary:
Introduction. Despite their therapeutic benefits, almost all drugs have adverse effects on various tissues and functions of the body, including the organs of vision, in which organ toxic reactions are easily detected. Many medications can lead to increased intraocular pressure in patients with risk factors and, as a result, cause an acute attack of angle-closure glaucoma. Purpose of the study. To study the effect of undesirable adverse drug reactions on the development of an acute attack of angle-closure glaucoma. Evaluate pestilence experience on this issue in scientific articles posted on the PabMed portal. Materials and methods. A number of scientific papers on drug-induced acute attacks of angle-closure glaucoma in elderly patients were studied and analyzed on the PabMed portal. Results. An acute attack of angle-closure glaucoma is an ophthalmic emergency characterized by optic neuropathy with associated visual field loss. One of the main risk factors for its development is increased intraocular pressure (IOP). Drug-induced glaucoma should be considered a form of secondary glaucoma because it is caused by specific systemic or local medications.Sulfonamide-based drugs (acetazolamide, hydrochlorothiazide, cotrimoxazole and topiramate) can cause an acute attack of angle-closure glaucoma due to swelling of the ciliary body and choroid. Glucocorticoids can impair the outflow of aqueous humor through the trabecular meshwork of Schlemm's canal due to physical and mechanical changes in the microstructure of the trabecular meshwork; cause deposition of various substances in the trabecular meshwork and inhibition of proteases and phagocytosis of trabecular endothelial cells. Traditional heart medications, including digitalis and quinidine, do not appear to have any effect on IOP. However, disopyramide phosphate appears to have some anticholinergic activity and may induce an attack of angle-closure glaucoma. Ipratropium bromide, an anticholinergic used to reverse bronchoconstriction, causes mydriasis and may precipitate an acute attack of angle-closure glaucoma in susceptible individuals. Conclusions. Drug-induced acute attack of angle-closure glaucoma is a dangerous complication of certain medications given to high-risk patients and may be associated with commonly prescribed medications (eg, influenza medications, antihistamines). Medically induced acute glaucoma is an important disease for both the ophthalmologist and general practitioner and is a potentially preventable cause of vision loss. Physicians prescribing such precipitating drugs should be aware of their potentially sight-threatening side effects in order to prevent, detect, and promptly consult with an ophthalmologist.
