ASSESSMENT OF PUBLIC SATISFACTION WITH THE ORGANIZATION OF AN APPOINTMENT WITH A DOCTOR IN MEDICAL ORGANIZATIONS OF THE SVERDLOVSK REGION

Summary:
Abstract. Making it possible to make an appointment with a doctor is a key indicator of access to care. Identifying the problems faced by both employees of medical organizations and the public when making an appointment with a doctor is an important task for executive bodies in the healthcare sector. Purpose - based on a comprehensive assessment of the satisfaction of patients and employees of medical organizations with the process of making an appointment with a doctor, identify problems and propose ways to solve them at the regional level. Materials and methods. In September 2023, 117 medical organizations conducted a voluntary anonymous online survey to assess satisfaction with the organization of making an appointment with a doctor. A total of 354 patients, 840 doctors, 349 receptionists, and 149 call center operators were interviewed. The survey was carried out using questionnaires developed by the State Autonomous Institution of Additional Professional Education «A.B. Blokhin Urals Institute of Health Care Management» for each group of participants, through Yandex forms via QR codes and links. Statistical processing was carried out using Microsoft Excel-2013 and descriptive statistics methods. Results. According to the survey results, 42,6% of patients, 29,1% of doctors, 18,7% of receptionists, 26,7% of call center operators are to some extent dissatisfied with the organization of making an appointment with a doctor. Leading problems on the part of patients and employees of medical organizations were identified: the work of the State Services portal, the registry, call centers, the lack of medical personnel, the speed of the Unified Digital Platform medical information system, and organizational recording problems. An action plan has been developed to improve the organization of making appointments with a doctor, a brief algorithm for the head of a medical organization. Discussion. Most of the problems associated with organizing an appointment with a doctor are similar in different regions of the Russian Federation. When developing areas for improving this process, it is important to take into account the peculiarities of the functioning of the medical information system in a particular region, as well as evaluate the available resources, including human resources. It is also necessary to take into account the peculiarities of the organization of primary health care. In order to make effective management decisions, regular monitoring of the satisfaction of all stakeholders is required, as well as the development and implementation of regional strategies to improve the work of the outpatient department and the organization of appointments with a doctor. Conclusion. Key areas for improving the system for making appointments with a doctor include: improving the operation of the medical information system, rational use of human resources, developing staff competencies in the context of digitalization of the healthcare system, as well as the transition to a patient-oriented model of outpatient care, focused on patient needs and based on the use of modern control technologies.
