FEATURES OF REGULAR MEDICAL CHECK-UP ORGANIZATION OF HIV-INFECTED PREGNANT WOMEN

Summary:
Background. Despite significant advances in treatment, HIV infection poses a serious threat to the general population health and remains a major public health problem. Prevention of mother-to-child HIV transmission is an urgent task of the Russian healthcare system. Purpose. To analyze statistical data on the coverage of prevention of mother-to-child HIV transmission among HIV-infected pregnant women in Moscow for the period 2018 – 2022. To summarize the features of regular medical check-up organization of HIV-infected pregnant women. Materials and methods. The data of federal statistical observation (Form № 61 «Information on the disease caused by the human immunodeficiency virus») for the Russian Federation and city of Moscow, medical records of patients receiving medical care on an outpatient basis (form 025/u-04) who applied to the Moscow City Center for AIDS Prevention and Treatment were studied. Results. The number of live births among HIV-infected women decreased by 16 % in Moscow in 2022 compared to 2021, which is also in line with the general trends, according to Federal State Statistics Service, of a decrease in the birth rate among the general population of Russia for the specified period. Since 2011, the number of HIV-infected women who became pregnant while on continuous antiretroviral therapy there has been increased by 59,7 %. Women who started antiretroviral therapy as prenatal preparation gave birth to healthy babies in 100 % of cases. From 2011 to 2021, the number of premature births in HIV-infected women decreased from 16,4 % to 11,2 % in Moscow maternity hospitals, and the mother-to-child HIV transmission risk decreased by more than 4 times. For the purpose of early detection of HIV infection in women during pregnancy and to improve the quality of medical care for HIV-infected pregnant patients, the responsibilities of an obstetrician-gynecologist of a maternity welfare center and an infectious disease doctor have been determined. Conclusion. Early detection of HIV infection in women for the purpose of subsequent lifelong follow-up and prescription of antiretroviral therapy, especially during pregnancy, and measures to prevent mother-to-child HIV transmission, will significantly reduce the incidence of HIV in newborns.
