PROBLEMS OF PATIENT COMPLIANCE AND THEIR SOLUTIONS IN RECOMMENDED SURGICAL TREATMENT OF CATARACTS AND ANTIANGIOGENIC THERAPY FOR CONCOMIENTED AGE-RELATED MACULAR DEGENERATION

Summary:
Introduction. The most common eye diseases among people 40 years of age and older are senile cataracts and age-related macular degeneration (AMD). These pathologies cause a gradual decrease in visual acuity in patients, which requires dynamic monitoring by an ophthalmologist and surgical treatment of cataracts and intravitreal administration of angiogenesis inhibitors for neovascular AMD. The most difficult option in the clinical practice of an ophthalmologist is the management of patients with senile cataracts and concomitant wet AMD. When a doctor decides on the highest priority direction of treatment in the presence of concomitant pathology, in some cases the problem of low compliance of patients with subsequent unfavorable prognosis for vision arises. Purpose. To analyze patient adherence to treatment and study proposed options for solving it for diagnosed senile cataracts and concomitant AMD in Russia and the world. Materials and methods. The materials for this review were published domestic and foreign scientific studies on the topic of adherence to the treatment of cataracts and wet AMD, as well as data from the World Health Organization (WHO), and clinical recommendations of the Russian Federation. We searched for available published articles using the following databases: elibrary, PubMed. Results. Today, diagnostic and treatment methods are becoming more advanced and accessible. However, many patients refuse recommended treatment for various reasons, causing a significant burden on the global economy. Analyzing the reasons for patients’ refusal of recommended treatment, we found that the low level of patient compliance is associated primarily with low interest in the established diagnosis, insufficient awareness of their disease, lack of financial opportunity, limited personal time, anxiety and fear. To improve the quality of medical care and improve patient adherence to treatment, some authors have proposed the introduction of rooms for monitoring the dynamics of the retina in patients with AMD, intravitreal administration of angiogenesis inhibitors directly on the day of consultation, as well as surgical treatment of cataracts in one-day hospitals. Conclusion. Thus, the improvement of medical and organizational approaches in the treatment of patients with the most common age-related ophthalmological diseases is demonstrated and predicted by an improvement in the functionality of the introduced methods, an increase in the volume and efficiency of operations performed, diagnostic measures, a decrease in the number of complications and a reduction in rehabilitation time and temporary disability of patients. Despite the availability of modern capabilities and proposed improved diagnostic and treatment methods, the problem of patient compliance remains not completely eliminated. Adherence to treatment requires further study and the introduction of new ways to solve this problem.
