FOLLOW-UP MEDICAL CARE OF PATIENTS WITH PSORIASIS IN THE RUSSIAN FEDERATION AND POSSIBLE APROACHES TO IMPROVE ITS ORGANIZATION

Summary:
Introduction. Psoriasis is a common, severe chronic skin disease. Its incurability dictates the need for medical care and dynamic monitoring throughout the patient’s life.
Aim. To assess the state of follow-up medical care of patients with psoriasis in the Russian Federation in 2015-2023 based on analysis of level and dynamics of rates, describing organization of follow-up medical care, and to develop proposals for its improvement.
Materials and methods. Based on federal statistical data we calculated follow-up medical care coverage rates of patients with psoriasis and follow-up medical care coverage rates for those newly diagnosed with psoriasis, total and in three age groups, in 2015-2023.
Results. In 2023 in the Russian Federation, 61% of patients with psoriasis were covered by follow-up medical care at the end of the year. The rate showed a slight upward trend in 2015-2023.
In 2023, 49% of patients with newly diagnosed psoriasis were taken under follow-up medical care in the same year. During the analyzed period, the rate decreased from 42% to 38% in 2015-2017, afterwards there was an increase of the rate in 2018-2023.
The dynamics follow-up medical care coverage in all age groups of patients demonstrates a low-intensity, gradual wave-like growth from the lowest level in 2015.
The greatest coverage by follow-up medical care of patients with newly diagnosed psoriasis is observed among children 0-17 years old, the lowest – among patients over working age. An upward trend of the rate is observed for adult patients of working age and those over the working age. The dynamics of the rate is unstable among children 0-17 years old with psoriasis.
Conclusion. Based on the level of analyzed rates, unaddressed problems in organization of follow-up medical care of patients with psoriasis in the Russian Federation can be stated. The coverage by follow-up medical care of all patients with psoriasis and of patients with newly diagnosed psoriasis is insufficient and does not reach target values among patients of all age groups. This indicates the need for an integrated approach for optimization of follow-up medical care organization. Conclusive statements: 1. Increase of follow-up medical care coverage rates in 2015-2023 is of low intensity. 2. The highest coverage rates are among children aged 0-17 years with psoriasis; the growth of rates is most intensive among patients of over working age. 3. There is a need for integrated approach to improving the organization of follow-up medical care for patients with psoriasis, including risk-based approach, modern organizational and technological options. 4. Prognostic models, developed based on data of the Russian Society of Dermatovenereologists and Cosmetologists can be used as tools for stratification of patient with psoriasis to groups of higher or lower risk of psoriatic arthritis. 5. Patient with psoriasis and a higher risk of psoriatic arthritis can be considered as priority group for proactive invitation for follow-up medical care, as well as target group for more active preventive measures. 6. Teledermatologic monitoring is acceptable and can be implemented effectively as part of follow-up medical care of patients with psoriasis.
