THE DEVELOPED MODEL OF THE ECOSYSTEM OF NEWBORN HEALTH PROTECTION IN THE WESTERN DISTRICT OF THE KRASNOYARSK TERRITORY WITH POSITIVE CHANGES IN THE MAIN DEMOGRAPHIC INDICATORS BASED ON THE RESULTS OF THE WORK ASSESSMENT FOR 2023

Summary:
Introduction. The analytical review describes the developed model of medical care at the territorial level, which led to the improvement of perinatal care for newborns. The analysis is carried out on the example of the efficiency of the launch and operation of the interdistrict Perinatal Center (hereinafter referred to as PC) in the Western District of Krasnoyarsk Territory (hereinafter referred to as WD KT).
Aim: To analyze the indicators that characterizes the improvement of perinatal care for newborns in WD KT, to evaluate the model of organization of child health care on the basis of the interdistrict Perinatal Center.
Material and methods. As a result of the study, scientific and practical data are reviewed, statistical information is studied, and author's conclusions are given.
Results. The analysis conducted for the city of Achinsk, Krasnoyarsk Territory (hereinafter KT) revealed that over the last 5 years there has been a decrease in the number of sick children or those who fell ill during the newborn period out of the total number of newborns born alive. Thus, in 2019 the number of sick and diseased newborns amounted to 1115 (42.1%) children, in 2020 - 1043 (39.8%), in 2021 - 1140 (39.3%), in 2022 - 959 (38.8%), in 2023 - 947 (39.0%) [4]. Over the same period, data analysis showed a steady downward trend in the total incidence indices for both WD KT and subject KT, with the rate of decline in the incidence indices in the correlated localities being 14.5% (616.7‰ in 2019 vs. 527.3‰ in 2023) and 16.3% (623.4‰ vs. 521.8‰, respectively), respectively. The reduction in the number of morbidities can be explained by the timely elimination of certain conditions occurring in the early neonatal period by 1.2 times and other diseases by 1.2 times, while the incidence of congenital fetal anomalies increased by 2.5%. The decrease in perinatal mortality rate for the analyzed time period was 15.94% in WD KT (6.9‰ in 2019 vs. 5.8‰ in 2023) and 19.18% for the subject - KT (7.3‰ vs. 5.9‰). In the structure of perinatal mortality about 4/5 is occupied by stillbirths, the level of which for the five-year period decreased by 0.8‰ (from 5.7‰ to 4.9‰) due to a decrease in the number of early neonatal deaths by 0.3‰ (from 1.2‰ to 0.9‰). Antenatal death prevails in the structure of stillbirth rate throughout the time, the share of which in 2023 is 3.33 times higher than the similar share of intrapartum death. Newborn mortality rates in the first week after birth had less variability in WD KT compared to perinatal mortality and stillbirth rates. Neonatal mortality decreased by 50% in Achinsk KT during the analyzed period. 1,257 visits were made by visiting teams of the neonatal resuscitation and counseling unit over 5 years. 185 children were transported to ventilator. 1,339 remote observations and telephone consultations were conducted. The number of transportation to Achinsk increased 1.4 times. In WD KT, after the change of structure and routing, in 2019 there were born children - 2 650, in 2020. – 2 623, in 2021 – 2 902, in 2022 – 2 472 and in 2023 – 2 476 children. More than 90% in Level III, the proportion of which increased by 4.4% (from 93.1% to 97.47%) in 5 years due to a decrease in the number of births in Level I facilities. Comparison of mortality rates of newborns during the first week of life by obstetric care facilities showed a decrease in the level in level III and level I facilities of the WD KT by 33.33% and 16.67%, respectively.
Discussions. The analysis of organizational aspects of medical care, supplemented by the data of medical documentation of the Perinatal Center, allowed us to identify the effectiveness of the built model of the health care ecosystem of the WD KT after the commissioning of the PC in Achinsk in the system of organization of comprehensive medical care for newborns.
Conclusions. In 2023, it was possible to reduce infant mortality both in the Western District of Krasnoyarsk Territory as a whole (up to 4.2‰) and on the basis of the Perinatal Center of KGBUZ “Krasnoyarsk Regional Center for Maternity and Childhood Protection No. 2” (3,2‰). The commissioning of the Perinatal Center in Achinsk and further improvement of personnel policy and applied technologies of highly specialized medical care, centralization of obstetric and neonatal services, anesthesiology and resuscitation, including visiting teams by expanding the network of level III obstetrics facilities, made it possible to reduce the time for pregnant women to reach the perinatal center and save children's lives.
