Traumatism is not only a medical problem but also has significant socio-economic implications, as it leads to a high level of disability and medical-social consequences: disability and mortality, while influencing demographic factors. The level of traumatism in Russia has fluctuated over recent years, though there is an overall trend towards its reduction. Domestic trauma in Russia is a serious social issue affecting thousands of families and burdening the state with significant costs. According to statistics, the most vulnerable groups of the population are elderly individuals, who most often become victims of accidents in domestic settings. The causes of trauma are not only inattentiveness and negligence but also a lack of proper safety conditions.
Aim. To study the dynamics of domestic trauma among adults over working age in the Russian Federation from 2019 to 2023, analyze the trauma structure, investigate causes of death among the adult population, determine the main directions for the prevention of domestic trauma and its adverse consequences, and develop a concept for increasing life expectancy in the Russian Federation.

Results. Mortality from external causes remains the main indicator of trauma. However, the scale of the problem and its burden on society are formed from the overall epidemiological picture of trauma that required treatment in outpatient and inpatient settings, as well as fatal outcomes resulting from injuries, poisonings, and other external factors.
Conclusion. The level of trauma in Russia remains a significant problem that requires a comprehensive approach. Domestic trauma in Russia is a complex issue that demands a multidisciplinary approach. There is a need for efforts from both the state and society to reduce trauma levels and enhance safety and quality of life for citizens. Analyzing the causes and consequences of trauma should be at the forefront for developing effective strategies and programs for its reduction.
