ORGANIZATIONAL ASPECTS OF PROVIDING ASSISTANCE TO PATIENTS WITH ETHANOL POISONING IN AN EMERGENCY DEPARTMENT

Introduction. Excessive alcohol consumption is a risk factor for disability and increased mortality. The difficulty in choosing a unit to provide assistance to patients with an alcoholic factor is that alcohol intoxication can mask somatic pathology, including severe one. It is necessary to improve the organization of care at the stage of the emergency department.
Purpose of the study. Assessment of the main aspects of the organization of care for patients diagnosed with “Acute ethanol poisoning” at the stage of the inpatient emergency department of the St. Petersburg Research Institute of Emergency Medicine named after. I.I. Dzhanelidze" for the period 2018-2022.
Materials and methods. We analyzed data of more than 40 thousand patients hospitalized at the St. Petersburg Research Institute of Emergency Medicine named after. I.I. Dzhanelidze in 2018-2022. We used statistical and analytical methods.
Results. The study examined the volume of patient admissions by years and months; a downward trend was noted only in 2020. The maximum flow was observed between 21:00 and 24:00. More than half of the patients (50.6%) are admitted in a satisfactory condition. Patients with antisocial behavior make up 40.3-89.0% in groups of varying severity. The largest number of admitted patients with a diagnosis of "Acute ethanol poisoning" are hospitalized in acute poisoning departments, a significant number of patients do not need (6.6%) hospitalization or refused treatment (11.6%). There is an increase in the number of patients hospitalized in the toxicology intensive care unit, which complies with the requirements of the Order of the Health Committee of St. Petersburg dated August 31, 2020 No. 625-r on referral to the toxicology center of the St. Petersburg Research Institute of Emergency Medicine named after I.I. Dzhanelidze is the most severe contingent of patients. More than 2/3 (64.6%) of the incoming flow with an alcohol factor received the necessary and sufficient amount of care in the conditions of the emergency department (without hospitalization in a 24-hour bed) and were discharged for outpatient treatment.
Conclusion. The flow of patients with an alcohol factor requires significant resources of a medical organization. It requires special attention both in connection with the difficulties of the diagnostic process, often masked by alcohol, and behavioral disorders, the asocial appearance of patients. It should certainly be isolated from the flow of sober patients to avoid conflict situations. It is important to have territorially isolated waiting and medical care areas, isolation wards, surveillance cameras, constant medical supervision, security services and specially trained personnel.
