DYNAMICS OF GENERAL MORBIDITY AND MORTALITY OF THE ADULT POPULATION FROM DISEASES OF THE CIRCULATORY SYSTEM

Summary:
Introduction. The health situation of the Russian population remains difficult due to the high mortality rate and low birth rates over a sufficiently long period of time. For many years, diseases of the circulatory system (DCS) have been one of the most serious public health problems both in the world and in Russia, significantly affecting the morbidity and mortality of the population. Purpose: to study the current trends in the general morbidity and mortality of the adult population from diseases of the circulatory system. Materials and methods. The indicators of the general morbidity and mortality of the adult population of Novouybyshevsk, Samara region, for the period from 2019 to 2023 were studied. The scientific base of the study is the state budgetary healthcare institution Novokuibyshevsk Central City Hospital (NCCH), serving the population of Novokuibyshevsk with an average annual population for 2019-2023 equal to 101.4 thousand inhabitants. The official data of the Samara Regional Medical Information and Analytical Center (MIAC) were used (form No. 7 "Structure of mortality from individual causes by sex and place of death" of the automated system "Mortality"), as well as the Territorial Body of the Federal State Statistics Service for the Samara Region (Samarastat) (in terms of population size and structure) for 2019-2023. In the course of the study, the reporting form of Federal Statistical Observation No. 12 "Information on the number of diseases registered in patients living in the service area of a medical organization" was also studied. The relative and average values of morbidity and mortality of the population were calculated, statistical and analytical research methods were used. Results and discussion. In the dynamics of 2019-2023, the total incidence of diseases of the circulatory system in the adult population increased from 470.2 cases per 1,000 adults to 559.1 per 1,000, or by 18.9%. The overall incidence of diseases characterized by high blood pressure increased the most, from 186.1 to 244.0 per 1,000 people 18 and older, by 31.1%. In the dynamics of the structure of the general incidence of BSC over five years, an increase in the proportion of diseases characterized by high blood pressure can be noted due to a slight decrease in the proportion of coronary heart disease and other diseases of the circulatory system. The mortality rate of the adult population in 2019-2023 decreased from 59.3 to 56.3 per 10,000 inhabitants, or by 5.1%. To the greatest extent, there is a decrease in adult mortality from cerebrovascular diseases, which amounted to 23.6%, respectively, from 25.4 to 19.4 cases per 10,000 population. Ischemic heart diseases (45.1%) and cerebrovascular diseases (34.5%) are the leaders in the structure of adult mortality from DCS. Against the background of a decrease in adult deaths from all causes of DCS in 2023, there is an increase in deaths from pericarditis, endocarditis, valve lesions and conduction disorders as a result of complications after coronavirus infection. Conclusion. Diseases of the circulatory system have pronounced indicators of general morbidity and mortality among the adult population. Among the adult population over the working age, the rates of general morbidity from DCS are on average more than twice as high as those among the adult population. The structure of the overall incidence of DCS among people older than able-bodied is characterized by a lower proportion of diseases characterized by high blood pressure and a higher proportion of cerebrovascular diseases and coronary heart disease. Among the adult population over the working age, the mortality rates from DCS are on average 2.5 times higher than similar values among the adult population, both for the entire class and for individual diseases, while over five years the mortality rate has increased by 4.4% against the negative mortality rate among the adult population as a whole (-5.1%). The structure of mortality from DCS among people older than working age is close to a similar structure of mortality in the adult population as a whole.
