NEW ALGORITHMS FOR NURSING CARE OF PATIENTS WITH ACUTE CEREBRAL CIRCULATION ACCIDENT (AUTHOR'S DEVELOPMENTS)

Summary:
Introduction: Vascular diseases of the brain occupy one of the leading positions in the structure of mortality and disability of the population from diseases of the circulatory system. Complications of acute cerebrovascular accident (ACVA) are diverse in their composition: speech and vision disorders, bedsores, aspiration pneumonia, etc. One of the main factors in the fight for the life and health of the patient is considered to be timely and competently organized nursing care. One of the ways to improve nursing care for patients who have suffered ACVA is the introduction of new algorithms for the work of mid-level medical personnel. Purpose of the study – to propose new approaches to nursing care for patients with acute cerebrovascular accident in a hospital setting. Materials and methods: The base of the study is the neurological department № 1 of the St. Petersburg State Budgetary Healthcare Institution "City Multidisciplinary Hospital № 2". In the process of implementing new algorithms for nursing care for patients with stroke, the staff of the neurological department № 1 read many literary sources on the relevant topic, used many years of experience working with patients of this profile, taking into account all the pros and cons of the generally accepted practice of nursing care. Results and discussion: One of the main tasks of the neurological department staff is not only the timely detection of bedsores, but also a competent assessment of the risk of their development. In this regard, the procedure for mandatory completion of the "Sheet of Nursing Assessment of the Risk of Development and Stages of Bedsores", "Sheet of Registration of Anti-bedsore Measures", "Card of Observation and Treatment of a Patient with Bedsores" was introduced into the work of the neurological department № 1 of City multi-field hospital № 2. As a result, the registration of bedsores in patients with acute cerebrovascular accident of the neurological department № 1 increased threefold, it was possible to increase the registration of bedsores at the first stage threefold, reducing their registration to zero at the last, fourth stage and to a minimum at the third stage. In order to avoid the development of various complications and to check the effectiveness of the work, the department staff compiled a "Patient Questionnaire" that is filled out by the patient himself. Conclusion: Practice shows that the introduction of new approaches to nursing care for patients with acute cerebrovascular accident brings its benefits. As a result of the algorithms introduced in the neurological department № 1 of City multi-purpose hospital № 2, the number of registered bedsores in patients with acute cerebrovascular accident increased, the number of patients with stage 1 bedsores increased, the detection of stage 4 bedsores was reduced to zero, and stage 3 bedsores were reduced to a minimum. The new algorithms improved the work of the nursing staff, which was noted by the patients themselves. The authors hope that the algorithms considered will be evaluated in practice by doctors and nurses of the corresponding specialization in other medical institutions.
