DEVELOPMENT OF ALGORITHMS FOR PROVIDING NURSING CARE TO PATIENTS WITH ACUTE CEREBRAL CIRCULATION ACCIDENT

Summary:
Introduction: Acute cerebrovascular accidents (ACVA) are the second most common cause of death and disability among the population due to circulatory diseases. The most common complications of ACVA are cognitive impairment, bedsores, aspiration pneumonia, etc. Modern diagnostic and endovascular surgery methods, as well as innovations in the pharmaceutical field, can dramatically reduce the risk of complications and subsequent disability of patients who have suffered ACVA. However, the main factor in the fight for the life and health of the patient remains timely and competently organized nursing care. Purpose of the study – development of algorithms for providing nursing care to patients with acute cerebrovascular accident. Materials and methods: The study was conducted at the Neurology Department №1 of City Multidisciplinary Hospital №2, where patients with suspected stroke are admitted daily. A step-by-step study was conducted: 1) a comparison of the number of complications in patients with stroke between 2019 and 2022 was carried out, the risk of developing bedsores according to the Waterloo scale and the stages of their development were assessed; 2) self-care and independence were assessed in patients born in 2022 according to the Rivermead Index; 3) a survey of 20 patients with stroke in 2022 was carried out using a specially designed questionnaire to assess the presence of functional disorders; 4) a survey of 20 patients with stroke was conducted in 2023 to check the effectiveness of the measures taken to improve the nursing care provided. Results and discussion: The most common complications in patients with stroke in 2019 and 2022 were: pneumonia, pressure ulcers, urinary tract infections. In 2022, the number of patients with pressure ulcers increased due to the launch of testing for the risk of developing pressure ulcers according to the Waterloo scale. This testing made it possible to identify more patients with pressure ulcers at an early stage in 2022 compared to 2019 and treat them in a timely manner. The percentage of patients discharged with a high level of independence and self-care (according to the Rivermead scale) is high - 63%, indicating a high level of medical and rehabilitation care for patients by the staff of the neurology department. In the age group of 80 - 95 years, the presence of various disorders is observed naturally more often than in the age groups of 30-60 and 60-80 years. A survey conducted in 2023 showed positive dynamics in the work of mid-level medical personnel and a high degree of patient satisfaction with the nursing care provided. Conclusion: The conducted work shows that well-organized nursing care for patients with acute cerebrovascular accident allows: to improve the quality of medical care, to reduce the risk of complications, falls and injuries of patients, to improve the positive emotional background of patients, to prevent the manifestation of depressive states. The applied method of working with patients can be implemented in any department of a hospital with a wide variety of nosologies.
