IMPROVEMENT OF EARLY DIAGNOSIS AND PREVENTION PROGRAMS OF CKD WITHIN THE FRAMEWORK OF DISPENSARY EXAMINATION OF POPULATION GROUPS OF MOSCOW FROM THE POSITION OF PATIENT-ORIENTEDNESS

Summary:
The relevance of the study is determined by the fact that chronic kidney disease (CKD) is a common pathology, diagnosed in 10-13% of the adult population and more than 20% of them are people over 60 years old. The population of patients with multimorbidity and concomitant polypharmacy is at increased risk of acute and chronic kidney diseases. The actual incidence may be significantly higher due to insufficient diagnostics and awareness of the population.
Objective of the study: to substantiate the improvement of early diagnosis and prevention programs for CKD within the framework of clinical examination of population groups.
The study materials were sources of scientific literature of recent years, as well as statistical data on patient diagnoses and laboratory diagnostic results based on the database of Moscow city polyclinics.
Results. Early diagnosis of CKD is important for the healthcare system both in terms of managing the risks of endocrinological and cardiovascular diseases, preventing the development of late stages of CKD, and in order to reduce direct and indirect costs required for the treatment and rehabilitation of patients. In 2023, 47,011 diagnoses of CKD, hypertension and diabetes were established in Moscow city polyclinics, which clearly illustrates the relevance and scale of the problem under consideration. In order to improve the early diagnosis of CKD in the outpatient healthcare sector of Moscow, screening algorithms are being developed aimed at an in-depth examination of patients from risk groups. These patients are recommended to undergo laboratory tests that are early markers of CKD: determination of albumin/protein in urine, determination of SCF based on measuring the level of creatinine in the blood serum. Thus, in the outpatient link of Moscow in 2023, 294 thousand studies of CKD markers were performed.
Discussion. In order to improve diagnostics and timely detection of CKD at early stages in the outpatient link of the city of Moscow, it seems advisable to amend the procedure for conducting preventive medical examinations and medical examinations of certain groups of the adult population. The implementation of these measures will facilitate the detection of abnormalities in renal function and establish a diagnosis of CKD at an early, preclinical stage, which will reduce the burden, progression and mortality from CKD in the city of Moscow.
Conclusion. Analysis of modern medical literature and experience in improving early diagnosis of CKD in the outpatient sector of Moscow demonstrate that taking into account risk factors, properly organized diagnosis and treatment of individuals from high-risk groups are necessary components of personal management of patients with CKD.
