Summary:
Relevance. Currently, there is an increase in the incidence of children in the field of otolaryngology, while many authors note the low availability of ENT doctors in the outpatient and polyclinic link. At the same time, there was no comprehensive study of the effectiveness of the work of an otorhinolaryngologist; there was no availability of specialized assistance among the child population, which determined the relevance of our study.
The aim of the study: to conduct an analysis of the effectiveness of the work of an otolaryngologist in carrying out preventive medical examinations of minors, in comparison with the provision of specialized medical assistance for the disease.
Materials and methods. A group of scientists from Novosibirsk State Medical University, together with medical organizations in Novosibirsk, analyzed the work on providing specialized medical aid in the field of «Otolaryngology» to the child population from 2018 to 2023. As part of the study, a regulatory analysis was carried out, as well as an analysis of statistical data from Rosstat and the Novosibirsk Region.
Results and discussion. The share of children with functional impairments and chronic diseases in Russia is 56%, in the Novosibirsk region - 42%, which increases the need for specialized medical assistance. Analysis of the structure of the workload of an ENT doctor for the study period showed that 81% of the working time is taken up by preventive appointments, and visits to an otolaryngologist for illness are 19%. At the same time, the results of preventive examinations showed that, despite the high prevalence of chronic pathology among the child population, 0.2% of children needed stage 2.
Conclusions. Data analysis shows an annual increase in morbidity among children in the «Otolaryngology» profile. In Russia, an increase in respiratory diseases by 18.4% was registered, and in the Novosibirsk Region - by 27.6%. There is a stabilization of morbidity from diseases of the ear and mastoid, in Russia the level of such pathology has decreased by 7.1%, however, in the Novosibirsk Region an increase of 8.7% was registered. High levels of morbidity among children in the field of otorhinolaryngology indicate insufficient availability of specialized medical aid. A high level of coverage of children with preventive medical examinations is noted. During the observation period, there was an increase in preventive examinations by 3.8% in the Russian Federation, and by 40.1% in the Novosibirsk Region, which is associated with the active development and observance of preventive measures aimed at the child population, both at the federal and regional levels. The identified high involvement of otolaryngologists in conducting preventive examinations affects the insufficient availability of specialized aid and the growing need from the child population. In order to reduce the incidence of otolaryngology among children, it is important to ensure the availability of an appointment with an otolaryngologist when a disease occurs.
