RESULTS OF THE SURVEY OF OPHTHALMOLOGISTS ON AWARENESS AND CONCERN REGARDING EARLY DETECTION OF DIABETIC RETINOPATHY AND ITS DIGITAL SCREENING IN THE TOMSK REGION

Abstract. The implementation of programs for the prevention of diabetic retinopathy—the leading cause of blindness among working-age individuals worldwide—begins at the level of primary healthcare. Therefore, the awareness of primary healthcare physicians regarding early diagnosis, complications, outcomes, and risk factors significantly determines the overall success of such initiatives. It is crucial for physicians involved in prevention programs to possess up-to-date knowledge and competence. Surveys allow for the examination of physicians' practices in screening and referring patients with diabetic retinopathy. Studying the knowledge and concern of ophthalmologists about early detection of diabetic retinopathy and evaluating their subjective opinions on healthcare organization will help identify areas for further improvement in the digital infrastructure of ophthalmology. This work presents the results of the survey of ophthalmologists in the Tomsk region. Materials and Methods: сompleted questionnaires from 62 ophthalmologists in the Tomsk region, including 34 mandatory questions. The survey was conducted during the first and second quarters of 2024. The analysis of the survey results was performed using descriptive statistical methods. Data processing was carried out using "MSOffice Excel 2010" spreadsheets. Results. The study highlighted the necessity of implementing a digital infrastructure in the Tomsk region, which includes establishing a Retinal Pathology Center, adapting patient routing for retinopathy, and improving digital communication among healthcare organizations, with a key element being a neural network platform based on artificial intelligence. According to the physicians, the main outcome of screening is the prevention of complications from retinopathy and the preservation of visual functions (96.7% of responses) through timely treatment (92.4%) and, as a consequence, prevention of blindness and disability (86.7%), attributed to timely diagnosis of retinopathy (83.3% of responses). The majority of practicing physicians (76.7%) positively assess the need for digital screening, 90.0% of respondents emphasize the importance of an effective routing scheme, and 76.7% support the organization of a Retinal Pathology Center. There is an identified need for professional further training for at least 26.7% of specialists providing care to patients with retinopathy, as well as confirmation of a substantial workload on ophthalmologists at 70.0%. Conclusions. Conducting a survey among ophthalmologists is a tool that allows for assessing the quality of medical care and making appropriate management decisions.
