PROVISION OF URBAN AND RURAL POPULATION WITH MEDICAL PERSONNEL IN THE PERIOD FROM 2015 TO 2023

Summary:
Introduction. Provision of medical personnel is one of the key pillars in the organization of health care, as the timeliness and quality of medical care depends on it.
Objective. To study the current situation in the provision of rural and urban population with medical personnel on the example of the Udmurt Republic.
Materials and methods. We analyzed the data on the provision of medical personnel and nursing staff in the Udmurt Republic in comparison with the indicators for the Volga Federal District and the Russian Federation as a whole for the period from 2015 to 2023, presented in the official statistical materials of the Ministry of Health of Russia on resource provision in 2017, 2019, 2021 and 2023 and 2024. Analytical and statistical methods of research (calculation of extensive and intensive indicators) were used in the work. Spreadsheets “MSOffice Excel 2010” were used for data processing.
Results. The highest provision of the urban population with medical personnel in the Volga Federal District, as well as in the country as a whole, coincided with the beginning of the Covid-19 pandemic (45.43 and 45.84 per 10,000 urban population, respectively, in 2020), while in the Udmurt Republic the indicator was higher than in the Volga Federal District and in the country as a whole by 13.9% and 13.2%, respectively. Over the period under study, the supply of doctors in the city increased, including in the Volga Federal District and the country as a whole by 2.1% and 2.0%, respectively, in the Udmurt Republic it decreased by 7.1%, despite this, the supply of urban population with medical personnel in the Republic remains above the district and national average. The highest provision of the rural population with medical personnel both in the Udmurt Republic and in the Volga Federal District and the Russian Federation as a whole was observed at the beginning of the period under study; in subsequent years the indicators were unstable. Irrespective of the Covid-19 pandemic, the supply of physicians in rural areas decreased during the study period, including in the Udmurt Republic (by 14.7%), in the Volga Federal District (by 11.0%) and in the country as a whole (by 7.9%). In the Udmurt Republic, despite more pronounced negative dynamics, the indicator remains above the district and national average. Availability of nursing staff in the city and in rural areas was the highest at the beginning of the study period, during the Covid-19 pandemic (c 2019-2022) in the city decreased, including in the country as a whole by 5.6%, in the Volga Federal District - by 5.5%, in the Udmurt Republic - by 6.7%, in rural areas there was a decrease by 8.0%, by 7.5% and - by 10.7%, respectively. For the whole period under study, the availability of middle medical personnel in the city decreased by 14.1% for the country as a whole, in the Volga Federal District - by 12.5% and in the Udmurt Republic - by 25.8%, in rural areas - by 14.8%, 17.4% and 24.0%, respectively.
Discussion. The growing share of older medical personnel and partial outflow of specialists to non-state medical organizations are reflected in the dynamics of the supply of specialists, which requires a managerial decision to increase the inflow of young personnel into the public health care system.
Conclusion. The decrease in the supply of urban population with medical personnel during the Covid-19 pandemic did not affect the dynamics of indicators in the Volga Federal District and in the country as a whole. In the Udmurt Republic, despite the decrease in the supply of doctors, its level remains above the national and district average both in urban and rural areas. Provision with average medical personnel, despite the decrease in the indicator, in rural areas remains above the county and the national average, in the city - slightly below the county level, but above the national average.
